高频超声引导锁骨下静脉精确置管术在危重患者中的应用

目的：探讨超声引导锁骨下静脉穿刺精确置入中心静脉导管术在在危重患者中的应用的价值。方法：将2008-01-2014-01在我院治疗需要留置中心静脉导管的500例患者随机分成超声组（超声引导下锁骨下静脉精确置管）245例和对照组（传统锁骨下静脉穿刺法置管）255例,比较两组患者一般临床特征、穿刺成功率、并发症发生率。结果：两组患者性别、年龄、APACHEⅡ评分、入院诊断差异无统计学意义（P〉0.05）。与对照组比较,超声组穿刺成功率明显提高,而尝试进针次数和并发症明显减少（P〈0.05）。结论：超声引导锁骨下静脉穿刺精确置管成功率高,并发症少,值得临床推广。

The application of high frequency ultrasound-guided subclavian vein accurate catheterization operation in critically ill patients

Objective：To explore whether it is valuable to make the application of ultrasound-guided subclavian venous catheterization operation inserted in central venous puncture precisely in critically ill patients. Method： 500 patients needed to be treated central venous catheter from 2008 January to 2014 January in our hospital were ran- domly divided into ultrasound group （245 cases of ultrasound-guided subclavian vein catheterization ） and control group （255 cases of traditional subclavian vein puncture catheter）. Compare general clinical characteristics,puncture success rate,complication rate of two groups. Result：There was no significant difference of two groups in sex, age, APACHE Ⅱ score, diagnosis （P〉0.05）. Compared with the control group, the ultrasound group significantly increased successful puncture rate,while decreased trying puncture times and the complications （P〈0.05）. Conclusion： High frequency ultrasound guided subclavian vein accurate catheterization operation has high success rate, less complications, which is worthy of clinical application.
