项目管理在血液透析患者肾性骨病性疼痛护理中的应用效果

目的观察项目管理在血液透析患者肾性骨病性疼痛护理中的应用效果。方法选取2016年3月至2017年2月在湛江中心人民医院行维持性血液透析并发肾性骨病性疼痛的患者46例作为对照组,另选取2017年3月至2018年2月的46例同病种患者作为观察组。对照组实行常规护理,观察组在常规护理的基础上实施项目管理。对比两组护理前、护理6个月后血磷、血钙、甲状旁腺激素水平的变化及疼痛程度、舒适度、心理状况、生活质量的变化。结果实施项目管理后,观察组血磷、血钙、甲状旁腺激素水平均显著低于对照组,差异有统计学意义(均P<0.05)。两组护理前疼痛程度、舒适度、心理状况及生活质量得分比较差异均无统计学意义(均P>0.05);观察组护理后的疼痛程度、舒适度及心理状况得分均显著低于对照组,差异有统计学意义(均P<0.05)。观察组护理后的生活质量得分显著高于对照组,差异有统计学意义(P<0.05)。结论运用项目管理模式可有效缓解血液透析肾性骨病患者的疼痛问题,并改善舒适度和心理状况,提高患者的生活质量。

Application effect of project management in nursing care of patients with renal osteodystrophy pain in hemodialysis patients

Objective To observe the effect of project management in nursing care of patients with renal osteodystrophy pain in hemodialysis patients.Methods Forty-six patients who underwent maintenance hemodialysis complicated with renal osteodystrophy pain in Zhanjiang Central People's Hospital from March 2016 to February 2017 were selected as the control group,and another 46 patients with the same disease from March 2017 to February 2018 were selected as the observation group.The control group received routine nursing,while the observation group implemented project management on the basis of routine nursing.The levels of serum phosphorus,serum calcium and parathyroid hormone before and after 6 months of nursing were compared,as well as the changes in pain degree,comfort level,psychological status and quality of life of the two groups.Results After project management,serum phosphorus,serum calcium and parathyroid hormone levels in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.05).There were no significant differences in the scores of pain degree,comfort level,psychological status and quality of life between the two groups before nursing(P>0.05).The pain degree,comfort level and psychological status score of the observation group after nursing were significantly lower than that of the control group,the difference was statistically significant(P<0.05).The quality of life score of the observation group after nursing was significantly higher than that of the control group,the difference was statistically significant(P<0.05).Conclusions The project management model can effectively alleviate the pain of renal osteopathy patients in hemodialysis,improve their comfort and psychological status,and improve their quality of life.
