不同冰刺激治疗方案对脑卒中后吞咽困难的影响

目的：探讨不同冰刺激治疗方案对脑卒中后吞咽困难的治疗效果的差异，为探索合理的治疗方案提供依据。方法将105例脑卒中后吞咽困难患者按随机抽样法分为对照组（ A组）、单次治疗组（B组）和两次治疗组（C组）3组。所有患者均接受吞咽困难常规治疗，B组和C组再分别增加每日1次和2次的冰刺激治疗。观察3组患者吸入性肺炎的发生率和吞咽功能恢复率。结果 C组吞咽困难恢复总有效率为91．43％，高于A组和B组的54．29％，77．14％，差异有统计学意义（χ2＝12．86，P＜0．01）；C组吸入性肺炎发生率为11．43％，明显低于A组和B组的37．14％，20．00％，差异有统计学意义（χ2＝6．806，P＜0．05）。结论每日2次的冰刺激治疗更有助于改善吞咽功能，并可有效预防吸入性肺炎的发生。

Effect of different methods of ice stimulation on patients with post-stroke dysphagia

Objective To observe the effectiveness of different ice stimulation treatment protocols on post-stroke dysphagia and provide basis for optimization of treatment protocols .Methods A total of 105 patients with dysphagia after stroke were randomly divided into three groups of control group （ group A ） , the single-session group （group B） and two-session group （group C）, respectively.All patients received conventional treatment of dysphagia , while patients of group B and C were additionally treated with ice stimulation once and twice a day, respectively.The incidence of aspiration pneumonia and recovery rate of dysphagia were observed and compared.Results The recovery of dysphagia in group C was with a total effective rate of 91.43%, which was significantly higher than those of group A （54.29%） and B （77.14%） （χ2 =12.86,P＆lt;0.01）.The incidence of aspiration pneumonia in group C was 11.43%, which was significantly lower than those of group A （37.14%） and B （20.00%） （χ2 =6.806, P＆lt;0.05）.Conclusions Ice stimulation for twice a day is more helpful in improving the post-stroke dysphagia , and it can effectively prevent the occurrence of aspiration pneumonia .
