认知行为疗法在小儿局麻下斜视矫正术中的效果研究

目的 探讨认知行为疗法在小儿局麻下斜视矫正术中的应用效果.方法 将选择局麻下行择期斜视矫正术的120例6～10岁的学龄期患儿分为两组,每组各60例.干预组采用认知行为疗法进行术前访视指导以及术中陪护.对照组采用常规方法进行术前访视指导,对比两组患儿在手术前及手术中的合作率、心率、血压和血清皮质醇的变化等.结果 干预组患儿进入手术室后血压、心率、血清皮质醇均低于对照组（P＜0.05）.结论 应用认知行为疗法对局麻下斜视矫正术患儿进行术前术中干预,能降低手术患儿的恐惧反应,提高患儿对局部麻醉的依从性和手术成功率.

Investigation of the effects of congnitive-behavioral therapy in pediatric strabismus surgery under local anesthesia

Objective To investigate the effects of congnitive-behavioral therapy in pediatric strabismus surgery under local anesthesia. Methods One hundrend an twenty children (between six and ten years), who were received strabismus surgery under local anesthesia, were randomly divided into two groups. congnitivebehavioral therapy group(group A, n=60) were treated with congnitive-behavioral therapy, while control group (group B, n=60) were treated with conventional preoperative guide. Tthe cooperation rate, heart rate, blood pressure and serum cortisol changes were compared between groups. Results Compared with group B, the heart rate, blood pressure and serum cortisol changes in group A were Significantly lower (x2=6.61, P＜0.01).Conclusions Preoperative application of cognitive-behavioral therapy for children with strabismus surgery under local anesthesia can significantly reduce children's fear reactions and improve compliance in children and the success rate of surgery.
