不同时间巴曲酶治疗对急性脑梗死患者脑功能及脑血流动力学的影响

摘    要:目的研究巴曲酶在不同时间应用对急性脑梗死患者治疗前、后的脑功能及脑血流动力学改变的影响。方法将50例发病12小时内的急性脑梗死患者,随机分为治疗A组和对照A组各25例;62例发病12~18小时患者,随机分为治疗B组和对照B组各31例。以上各组均予以丹参注射液20ml 生理盐水250ml,静脉滴注,每日1次。治疗组加用巴曲酶10U、5U、5U均加入250ml生理盐水中分3天予以静脉注射。治疗前后进行神经功能缺损评分,并测定患者脑电图、脑干听觉诱发电位(BAEP)和脑血流动力学变化。结果经巴曲酶治疗组的神经功能缺损评分、脑电图、BAEP和脑血流动力学变化均较对照组有明显好转,且12小时内使用巴曲酶者较12~48小时使用者改善更为明显。结论巴曲酶能有效治疗急性脑梗死,改善患者脑功能及脑血流动力学,尽早应用效果尤为明显。

Abstract:Objective To study the batroxobin effect in changing the brain functions and the emodynamics of acute cerebral infarction at different time.Methods Fifty patients with acute cerebral infarction in 12 hours were randomly divided into two groups, treatment group A and control group A, each had 25 patients;62 patients with acute cerebral infarction in 12-18 hours were randomly divided into two groups too: treatment group B and control group B,each had 31 patients. All groups were treated with salviae miltiorrhizae conpositae 20 ml in 250 ml normal saline(NS) intravenously. 10 U,5 U,5 U batroxobin was infused intravenously in treatment groups in 3 days, respectively.Electroencephalogram(EEG),brainstem auditory evoked potential(BAEP),brain thermodynamics were used at pre-therapy and post-therapy to evaluate the neurological deficites.Results The treatment groups were better than control groups in neurological deficites scoring,EEG,BAEP and brain thermodynamics.Furthermore,the effect was better when the batroxobin was used earlier(12 h vs 12-18 h). Conclusion Batroxobin is effective in treatment of acute cerebral infarction.It can reform the patients, brain function and hemodynamics,and should be used as early as possible.
