彩色多普勒对肾实质切开取石术的临床应用

本文检测31只肾多发或／和巨大铸型结石采用肾实质切开取石术，比较术前、术后15天、一月、三月血流动力学变化。肾结石组肾内血管床彩色血流信号稀疏或缺乏，肾动脉峰值流速（Vmax）和肾血流量（CO）明显低于对侧正常肾组（P＜0．05）。RI值两组变化不明显（P＞0．05），6例伴重度积水者RI值升高，与对照组比较差异显著（P＜0．05）。术后肾内彩色血流信号、Vmax和CO与术前比较无明显改善（P＜0．05）。伴重度积水者0．70＜RI＜0．80时，术中见患肾色泽正常，无坏疽征，术后15天RI值明显下降。认为彩色多普勒于术前即可判断患肾功能，并能预测术后患肾功能恢复状况。

Clinical Application of Color Doppler on Nephrolithotomy

Thirty-one cases with multiple or huge renal stone were ecamined by CDFI,and compared with the hemodynamic changes before and after nephrolithotomy for fifteen days ,one and three months,Before operation,the systolic peak velocity (Vmax),flow volume (FV) and color Doppler flow of stone group were significantly lower than that of the normal comtrol group (P>0.05) .The RI value was increased in six cases with severe hydronephrosis ,significantly different compared with that of the normal with those of pre-operation(P>0.05) .The color dippler flow ,Vmax and FV of post-operation weren't sigmificantly improved compared with those of pre-operation(P>0.05).The patients who had severe hydromephrosis and RI value from 0.7 to 0.8 ,were found to have mormal remal color and luster ,no gangreme sign during operation .Their RI values were sigmificantly decreased in fifteem post-operation day,The results suggested CDFI can not only monitor for pre-operational renal fumction,but also for follow up ecamination.
