二维及彩色多普勒超声对甲状腺癌的诊断价值分析

摘    要:目的:探讨二维及彩色多普勒超声对甲状腺癌的诊断价值。方法:对经手术病理证实的130例甲状腺癌及83例甲状腺腺瘤的二维及彩色多普勒超声进行比较分析。结果:甲状腺癌和甲状腺腺瘤在二维声像图、彩色血流丰富程度、RI及Vmax方面具有显著性差异(P<0.001,后两者P<0.0025)。钙化及低回声在甲状腺癌诊断中的特异性较高,分别为97.59%和86.75%,甲状腺癌的超声诊断符合率为80.77%;但微小癌及癌合并其它病变时,因缺乏特征性图像,误诊率高。结论:高频声像图中的钙化及低回声是诊断甲状腺癌的重要指标,而彩色血流丰富程度及频谱多普勒中的多项指标的变化将有助于提高甲状腺癌的诊断准确性。但对微小癌及癌合并其他病变的诊断,则需进一步探讨。

Abstract:Objective:To probe diagnostic value of two dimensional and Doppler color ultrasound for thyroid carcinoma.Methods:One hundred thirty cases with thyroid carcinoma and 83 cases with thyroid adenoma,confirmed by pathology,were analyzed comparatively with two dimensional and color Doppler ultrasound.Results:Significant differences in two dimensional image,more plentiful degree of color flow,higher resistance index(RI)and higher Vmax were observed in carcinoma group than those of adenoma of thyroid group.Specificities of calcification and low echoes were higher,97 59% and 86 75% respectively in carcinoma group.The accurate diagnostic rate of ultrasound was 80 77% for thyroid carcinoma.More misdiagnostic rate was present in small sized carcinomas or complications with other lesions because of lack of specific images.Conclusions:Calcification and low echoes in high frequency ultrasonography may be an important diagnostic indicator for thyroid carcinoma.More plentiful degree of color flow,higher Vmax or RI levels are helpful to improve the ultrasound diagnostic accuracy of thyroid carcinoma;however,ultrasound diagnostic value should be further investigated in small sized and complicated cacinomas.
