两种食管胃吻合口阻抗胃食管反流的对比观察

目的：探讨食管癌或贲门癌切除术后，用器械吻合与手工缝合食管胃吻合口的阻抗胃食管反流情况。方法：对115例贲门癌、食管癌切除术后两组患者行电子胃镜、食管压力测定、24h食管pH监测、放射性核素检查。结果：两组患者各种检查结果比较均无显著差异。结论：1)食管癌、贲门癌切除、食管胃吻合术后的患者广泛存在胃食管反流。2)食管胃两种吻合方法对预防反流无明显效果。24h食管pH监测应是检测胃食管反流最敏感的方法。

Contrastive Obseration of Preventing Gastroesophageal Reflux by Two Kinds of Esophagogastroanastomosis

Objective: To investigate which esophagogastro an astomosis by instrument or by hand is more able to prevent gastroesophageal refl ux(GER),in the patients after resetion of esophageal cancer or cardiac cancer. Methods: Two groups of 115 patients after resection of eso phageal cancer or cardiac cancer were studied by video-assisted gastroscopy, eso phagesl manometry,24h esophageal pH monitoring and radio-scintigraphy. Results:There were not significant differenes in the results of eve ry king of check between two groups of patients. Conclusion:1) There are GER in common in patients with the esophagogastroanastomosis afte r resection of esophageal cancer or cardiac cancer. 2) There are not obvious eff ects in priventing GER by the two kinds of esophagogastroanastomosis. 3) 24h pH monitoring is the most reliable method for detecting GER.
