脑动脉瘤的CTA和MRA及DSA对照分析研究

摘    要:目的:比较CTA、MRA、DSA三种方法诊断动脉瘤的优缺点,探讨非侵入性CTA和MRA取代DSA的可行性。方法:对收治的25例怀疑动脉瘤惠者同时作CTA、DSA检查,比较其影像学结果,对收治的22例怀疑动脉瘤惠者同时作MRA、DSA检查,比较其影像学结果。结果:CTA基本上可以发现大多数的动脉瘤。为后续的手术或者栓塞提供足够的影像学信息;MRA对较小动脉瘤(直径≤10mm)的诊断仍然存在一定难度,容易漏诊。结论:CTA可作为蛛网膜下腔出血以及可疑动脉瘤患者的初选检查方法,必要时再行DSA进行确诊。随着更多三雏重建技术的推广,其敏感性和特异性会更加提高,在不久的将来CTA可能会成为动脉瘤术前检查的首选。

Abstract:Objective To compare the advantages and disadvantages of CTA,MRA and DSA in the diagnosis of aneurysms,and to explore the feasibility of replacing DSA with non-invasive CTA and MRA.Methods 25 cases of suspected aneurysms were examined by CTA and DSA at the same time,and the imaging results were compared.22 cases of suspected aneurysms were examined by MRA and DSA at the same time,and the imaging results were compared.Results Most of the aneurysms could be found by CTA.To provide sufficient imaging information for subsequent surgery or embolization;MRA is still difficult to diagnose smaller aneurysms(diameter≤10mm),and it is easy to miss diagnosis.Conclusion CTA can be used as a primary examination method for patients with subarachnoid hemorrhage and suspected aneurysms.DSA can be used to diagnose subarachnoid hemorrhage and suspected aneurysms if necessary.With the popularization of more three-chick reconstruction techniques,its sensitivity and specificity will be improved.CTA may become the first choice for preoperative examination of aneurysms in the near future.
