肩胛上神经阻滞联合肌内效贴对脑卒中后肩痛的疗效分析

目的:初步观察肩胛上神经阻滞结合肌内效贴对脑卒中后肩痛患者上肢功能的疗效。方法:40例脑卒中后肩痛患者随机分为观察组和对照组各20例,对照组给予肩胛上神经阻滞加常规康复治疗,观察组在此基础上辅以肌内效贴治疗。比较2组患者治疗前后的疼痛视觉类比法(VAS)、Fugl-Meyer上肢运动功能评分(FMA-U)、改良Barthel指数(MBI)及肩关节被动活动度(PROM)。结果:治疗4周后,2组VAS评分较治疗前明显降低,上肢FMA-U评分、MBI评分及肩关节PROM明显较治疗前提高(均P0.05),观察组VAS评分、上肢FMA-U评分、MBI评分及肩关节PROM较对照组改善更明显(均P0.05)。结论:肩胛上神经阻滞结合肌内效贴可有效缓解脑卒中后患者肩部疼痛,对提高患者上肢功能及日常生活活动能力有重要的康复意义。

Effects of suprascapular nerve block with kinesio taping on the function of the upper limb in patients with shoulder pain after stroke

Objective: To observe the effects of suprascapular nerve block with kinesio taping on the function of the upper limb in patients with shoulder pain after stroke. Methods: Forty stroke patients with shoulder pain were randomly divided into experimental group and control group (n=20 each). The patients in the control group and experimental group were given suprascapular nerve block treatment and other conventional rehabilitation. The patients in the experimental group were given kinesio taping additionally. The visual analogue scale (VAS), the upper limb part of Fugl-Meyer Assessment (FMA-U), the Modified Barthel Index (MBI) and the shoulder passive range of motion (PROM) were used to evaluate the effectiveness before and 4 weeks after treatment. Results: After treatment for 4 weeks, the scores of VAS, FMA-U, MBI and PROM in both two groups were significantly improved as compared with those before treatment (P<0.05). Before treatment, the scores of VAS, FMA-U, MBI and PROM showed no statistically significant difference between the two groups, and those in the experiment group were significantly improved as compared with those in the control group after treatment for 4 weeks (P<0.05). Conclusion: Suprascapular nerve block with kinesio taping could effectively release the shoulder pain in patients with stroke, and it was beneficial to improve the function of the upper limb and activities of daily living.
