小脑顶核刺激对老年椎-基底动脉供血不足的影响

目的：探讨小脑顶核刺激对老年椎-基底动脉供血不足（VBI）的影响。方法：老年VBI急性期患者72例,分为药物组33例和综合组39例,均每日给予盐酸丁咯地尔200mg静脉滴注,综合组加以小脑顶核刺激,治疗前后经颅多普勒超声（TCD）检测2组患者双侧椎动脉（VA）、大脑后动脉（PCA）和基底动脉（BA）的平均血流速度（Vm）。结果：与治疗前比较,综合组治疗2-7d时,临床症状即出现明显改善（P〈0.05）;治疗4周后,2组患者双侧PCA、VA和BA的Vm均增加,综合组优于药物组（P〈0.05）。结论：小脑顶核电刺激可增加脑血流量,促进脑供血不足区域的血流灌注并对脑组织和神经元有保护作用,有助于老年VBI患者症状的早期改善。

The Effect of Fastigial Nucleus Stimulation on Senile Vertebral-basilar Insufficiency

Objective:To explore the effect of fastigial nucleus stimulation on senile vertebral-basilar insufficiency.Methods:Te data of 72 old patients suffering from vertebral-basilar insufficiency were collected,including their disease history,physical examination of their nervous system and CT,MRI of head.Then the patients were divided into two groups at random:drug treatment group and combined treatment group.The patients in the drug treatment group were subjected to intravenous injection of Buflomedil(200 mg),and those in the combined treatment group to intravenous injection of Buflomedil(200 mg)in combination with fastigial nucleus stimulation.The mean velocity of patients' basilar artery(BA)and two side vertebra arteries(VAs),posterior cerebral arteries(PCAs)were tested by transcranial Doppler(TCD)before treatment and 4 weeks after treatment respectively.The data were analyzed by SPSS10.0.Results:TCD showed that the mean velocity of the vertebrae-basilar artery system in both groups was less than that of normal population.After treatment,the symptoms of the combined treatment group were improved and the velocity of blood flow increased significantly at the early stage.The velocity of blood flow in combined treatment group was significantly increased as compared with that in drug treatment group.Conclusion:Fastigial nucleus stimulation can increase volume of cerebral blood flow,accelerate blood infusion to region of blood insufficiency,protect cerebral organ and nerves from injury and help to improve patients' symptoms early.
