全程糖尿病教育模式的构建、组织与管理

目的探讨全程糖尿病(DM)教育模式的构建、组织、管理与发展,并评价其实施效果。方法逐步建立了DM患者的门诊-住院-出院后教育、患者-护士-医生教育培训全程教育体系、患者信息资料数据库的建立与管理、糖尿病教育的研究等。结果8年来组织门诊教育讲座128次,听课人数达6310人次,组织病区集体教育362次,听课人数达5083人次。教育培训后患者的糖尿病知识评分、饮食、运动和药物治疗的依从性、自我血糖监测、自我管理能力均明显提高,血压和糖代谢控制明显改善(P<0.01)。结论门诊教育课程的设置为患者开辟了系统学习DM知识的课堂;住院教育是患者短期内系统学习、强化DM知识,提高自我管理能力的重要时机;热线电话咨询服务是对出院后及门诊的DM患者居家继续进行合理治疗、护理的便捷指导方式;糖尿病专科护士和基层医师的培训是推动糖尿病教育深入开展的坚实基础。

Construction,Origination, Management and Development of Diabetes Education Mode

Objective To probe construction, origination, man agement and development of the model of diabetes mellitus patients education. Methods Building up outpatients education, inpatients education a nd post-discharge patients eeucation, and building up patients, nurses and docto rs education system; Building up patients information management and doing diabe tes patients education research. We organized outpatients lectures 128 times and participants were 6310, inpatients lectures 362 times and participants were 508 3. Results The average knowledge test scores, dependence of diet, ex ercise, medicine treatment, self-monitor of blood glucose and self-care skill ac hieved excellently. Blood glucose and blood pressure improve better after educat ion (P<0.01). Conclusions Outpatients education courses provided a place of the diabetic knowledge for patients with diabetes. Inpatient education was a good c hance of learning diabetic knowledge and improvement of self-management ability. Consulting on line was a convenient way of instruction of the treatment and nur sing for the post-discharge patients with diabetes. Nurses and doctors training are principal measures of diabetes education.
