右胸及上腹两切口在食管中段癌手术中的应用

目的观察两切口术式行食管中段癌切除的优点和近期疗效及两种不同手术顺序的比较。方法对139例食管中段癌采用右胸后外侧加上腹正中两切口术式观察其优点及术后并发症情况,并进行两种不同手术顺序(开胸与开腹的先后)的比较。结果全组1例死亡0.7%(1/139),无切缘肿瘤细胞阳性、吻合口瘘及乳糜胸发生。结论食管中段癌采用右胸及上腹两切口术式具有切除率高、手术操作方便、术后并发症少及手术安全等特点。

Right thoracal and upper abdominal incisions for the treatment of mid-esophageal cancer

Objective To investigate the advantage and the curative effect in the near future of right thoracal upper abdominal double incisions for the therapy of mid-esophageal cancer and to compare the two different sequence of surgery. Method Right thoracal and upper abdominal double incisions was performed in 139 cases with mid-e- sophageal cancer. The advantages and the post-operative complications were evaluated, and compare the different se- quence of operation. Results Only one case died and the mortality was o. 7% （1/139）. All of the cases had no com- plications of the positive of cutting edge, the leakage of anastomosis and chylothrox. Conclusion Right thoracal and upper abdominal double incisions in mid-esophageal cancer was safe and had high dissection rate and convenient operative procedure, less post-operative complications.
