甲状腺癌声脉冲辐射力弹性成像特征分析

摘    要:目的 探讨甲状腺癌在声脉冲辐射力(ARFI)弹性成像声触诊组织定性成像(VTI)和声触诊组织定量(VTQ)成像上的特征.方法 分析20例甲状腺癌患者21个癌灶的ARFI成像特征.将甲状腺结节的VTI图像分为6级,评估甲状腺癌VTI图像分布特征.应用VTQ技术检测甲状腺癌及相同深度处周围甲状腺组织的横向剪切波速度(SWV)并进行比较分析.结果 21个甲状腺癌病灶VTI图像分析:1级0个;2级1个;3级2个;4级8个;5级7个;6级3个;VTI图像4级及以上占85.7%(18/21).甲状腺癌病灶VTQ检测SWV平均值为(4.70±2.79)m/s(范围:2.14~9.00 m/s);与其相同深度处周邻甲状腺组织的SWV平均值为(2.16±0.46)m/s(范围:1.37~3.33 m/s),两者比较差异有统计学意义(t=4.109,P<0.01);21个甲状腺癌病灶与相同深度处周邻甲状腺组织的SWV比值的平均值为2.23±1.43(范围:0.97~6.16).结论 AFRI技术、VTI和VTQ能定性及定量反映甲状腺癌病灶的硬度,可能对判断甲状腺癌性质提供诊断依据.

Abstract:Objective To investigate the acoustic radiation of force impulse (ARFI) characteristics of thyroid carcinoma, including virtual touch tissue imaging (VTI) and virtual touch tissue quantification (VTQ). Methods The ARFI characteristics of 21 thyroid carcinoma masses in 20 patients were retrospectively analyzed. The VTI imagines of thyroid masses were classified into 6-point levels. The shear wave velocities (SWV) of the thyroid carcinoma masses and the adjacent thyroid tissues at the same depth were detected using the VTQ technique. Results The VTI images of thyroid carcinomas were determined to be level 1 in none,level 2 in 1 lesion,level 3 in 2 lesion, level 4 in 8 lesions,level 5 in 7 lesions, and level 6 in 3 lesions. Eighteen of 21 ( 85.7% ) thyroid carcinomas had a VTI level 4 or higher. The average SWV value of 21 thyroid carcinomas was (4.70 ± 2.79 )m/s (range:2. 14 m/s to 9.00 m/s) and that of the adjacent thyroid tissues at the same depth was (2.16 ± 0.46 ) m/s ( range : 1.37 m/s to 3.33 m/s) , there was signifficant differences between them (t = 4. 109, P 〈 0. 001 ). The average SWV ratio of thyroid carcinomas to the adjacent thyroid tissues at the same depth was 2.23 ± 1.43 (range:0. 97 to 6.16). Conclusion The stiffness of thyroid carcinomas can be qualitatively or quantitatively measured by VTI or VTQ of the AFRI technique, which may provide diagnosis clues for thyroid carcinoma.
