联合检测脑钠肽、超敏C反应蛋白及D二聚体对急性心肌梗死的临床价值

摘    要:目的探讨联合检测脑钠肽(BNP)、超敏C反应蛋白(hs-CRP)及D-二聚体(D-D)在早期急性心肌梗死(AMI)中的临床意义。方法本试验随机入选了37例急性心肌梗死患者,应用微粒子捕捉酶促免疫发光法(MEIA)检测BNP、免疫透射比浊法检测hs-CRP及D-D,同时检测其他心肌梗死指标并行冠状动脉造影检查,并选取20例对照者(Ctrl组)做对比。结果①AMI患者的BNP、hs-CRP及D-D均明显高于正常组;②AMI组中KillipⅡ-Ⅳ的患者BNP及hs-CRP值高于KillipⅠ级的患者;③AMI患者中的BNP及hs-CRP水平与冠状动脉Gensini评分正相关;④AMI患者中的BNP与hs-CRP水平正相关。结论BNP、hs-CRP与D-D是AMI的敏感指标,联合检测可反映心功能及冠状动脉病变情况,可用于AMI危险分层。

Abstract:Objective We conducted a trial to explore the clinical significance of brain natriuretic peptide combined with hyper- sensitive C reactive protein and D-dimer in acute myocardial infarction.Methods We assessed profiles of brain natriuretic peptide, hypersensitive C reactive protein and D-dimer in 37 primary AMI patients complicated by the Gemini score compared with 20 healthy persons as contorl. The blood samples were obtained within 24 hours after the onset of AMI were separated by centrifugation immediately and they were tested within 12 hours under 4℃ .Results 1. The BNP, ks-CRP, D-D and CTn-I levels in AMI are all higher than in Ctrl group. 2. The BNP, hs- CRP andD-D levels in K Ⅱ-Ⅳ are all higher than in K Ⅰ group. 3. BNP and hs-CRP level are both related to Gensini Score. 4. BNP level is related to ks-CRP level. Conclusion brain natriuretic peptide, hypersensitive C reactive protein and D-dimer have high sensitivity in AMI patients and they are helpful to assess the state of AMI patients.
