低频电刺激联合盐酸多奈哌齐治疗血管性痴呆的疗效观察

目的探讨低频电刺激联合盐酸多奈哌齐对血管性痴呆（VaD）患者的治疗效果。方法将64例确诊为血管性痴呆的患者随机分治疗组（33例）及对照组（3l例），2组患者均在常规内科处理基础上给予盐酸多奈哌齐口服治疗，治疗组患者同时于双侧乳突区加用低频电刺激干预。治疗前及治疗12周后分别采用简易精神状态痴呆量表（MMSE）、临床痴呆量表（CDR）及Barthel指数评分进行疗效判定。结果经12周治疗后，2组患者的MMSE和Barthel指数评分均显著增高，CDR评分明显下降（P〈0．05），且以治疗组患者的改善幅度更为显著，与对照组比较，差异具有统计学意义（P〈0．01）；同时治疗组患者的定向力、注意力及记忆力评分均显著增加（P〈0．05或0．01），而对照组患者仅有记忆力评分较治疗前改善显著（P〈0．05）。结论低频电刺激联合盐酸多奈哌齐治疗VaD患者疗效显著，患者经治疗后其总体认知功能、日常生活活动能力及记忆力均显著改善。

The therapeutic effects of low frequency electrical stimulation associated with donepezil for vascular dementia patients

Objective To investigate the therapeutic effects of low frequency electrical stimulation (LFES) of the bilateral mastoid processes associated with donepezil in treating vascular dementia( VaD) patients. Methods Sixty-four VaD patients were randomly divided into a treatment group (33 cases) and a control group (31 cases). All patients were treated with donepezil ( 15 mg/d) as their basic, routine internal medicine treatment. The treatment group also received low frequency electrical stimulation of the bilateral mastoid processes. The therapeutic effects were assessed with the mini-mental state examination (MMSE) , the clinical dementia rating (CDR) and a Barthel index examination (BI) before and 12 w after treatment. Results MMSE and BI scores were improved in both groups, while CDR scores had decreased significantly at 12 w after treatment, especially in the treatment group. Orientation and concentration had significantly improved in the treatment group, as well as the memory scores in both groups. Conclusion The therapeutic effects of LFES associated with donepezil in VaD patients were obvious, and cognition, ability in daily living activities and memory can be improved.
