一次性骨髓腔输液连接装置的研制与临床应用

摘    要:目的探讨一次性骨髓腔输液连接装置的研制与临床应用效果。 方法选取山西医科大学第二医院急诊科2016年1至10月收治抢救的急危重患者172例,按照患者的入院时间顺序,将2016年1至5月我科收治抢救的90例危重患者作为对照组,骨髓腔穿刺成功后,连接输液器(输血器),并反复推注药物,将2016年6至10月我科收治抢救的82例危重患者作为观察组,骨髓腔穿刺成功后,连接一次性骨髓腔输液装置反复推注药物。比较观察组与对照组患者在延迟给药、骨穿针移位、骨穿针脱出、肢体肿胀、药液外渗等发生率的差异。 结果观察组在延迟给药、骨穿针移位、骨穿针脱出、肢体肿胀、药液外渗等方面的发生率明显低于对照组,差异均具有统计学意义(1.2% vs 11.1%,χ2=7.013,P=0.008;1.2% vs 7.8%,χ2=4.161,P=0.041;0 vs 5.6%,χ2=4.692,P=0.030;3.7% vs 14.4%,χ2=5.916,P=0.015;2.4% vs 15.6%,χ2=8.749,P=0.003)。 结论一次性骨髓腔输液连接装置保证了骨髓腔输液管路的密闭性和稳定性,防止穿刺针移位、脱出和药液外渗,提高了用药的安全性和有效性。

Development and clinical application of a disposable intramedullary infusion device

Abstract:ObjectiveTo investigate the clinical application of a disposable intramedullary infusion device. MethodsOf 172 critically ill patients who admitted in the Emergency Department of Shanxi Medical University Second Hospital from January 2016 to October 2016, 90 patients from January to May, 2016 were included in control group. In these patients, after successfullybone marrow puncture, they were connected to the transfusion device and repeatedly injected drugs. Another 82 critically ill patients from June to October, 2016, who were connected with disposable marrow cavity infusion device and repeatedly injected drugs after bone marrow puncture were defined as observation group. The differences of the incidence of delayed administration, needle displacement, prolapse, limb swelling, and fluid extravasation were compared between two groups. ResultsThe incidence of delayed drug administration, translocation, limb swelling, drug exudation in the observation group were significantly lower than that in the control group, respectively (1.2% vs 11.1%, χ2=7.013, P=0.008; 1.2% vs 7.8%, χ2=4.161, P=0.041; 0 vs 5.6%, χ2=4.692, P=0.030; 3.7% vs 14.4%, χ2=5.916, P=0.015; 2.4% vs 15.6%, χ2=8.749, P=0.003). ConclusionDisposable intramedullary infusion connection device can ensure the tightness and stability of intramedullary infusion pipeline, prevent needle migration, prolapse and extravasation, and improve the safety and effectiveness of medication.
