FLAIR序列鉴别脑基底节区腔隙性梗塞与血管周围间隙的初步研究

摘    要:目的 探讨FLAIR序列在鉴别脑基底节区腔隙性脑梗塞与血管周围间隙的可行性。方法 回顾分析50例腔隙性脑梗塞及30例非脑血管病变对照组病人脑部MRI资料,MRI检查包括轴位SET1WI及FLAIR成像,轴位及矢状位快速自旋回波(TSE)T2WI。结果 50例腔隙性脑梗塞病人脑基底节区表现为多发点状病灶,SET1WI呈低信号改变,SET2WI呈高信号,FLAIR序列表现为点状高信号30例,病程均在1个月内;中心呈低信号,周围包绕环行或半环行高信号9例,病程均长于1个月;余11例病灶具胩述两种病灶特征,病程为1年内反复发作,30例对照组病人见基底节区小点状灶,MRI表现为SET1WI及FLAIR序列呈低信号改变,SET1WI呈高信号,周围脑组织正常。结论 我们认为FLAIR序列可以鉴别脑基底节区腔隙性脑梗塞与血管周围间隙。

Abstract:Objective It' s a study of the feasibility of FLAIR s equence in differentiating lacunar stroke from Virchow -Robin space.Method Brain MRI were retrospectively studied in 50cases of lacunar stroke and 30normal people as the control group.MRI exam ination included SET 1 WI and FLAIR sequence at transverse s ection and TSE T 2 WI at transverse and sagittal sectio n.Result In all lacunar stroke cases multiple spot lesions at ganglion basilar areas were demonstrated hypointensed on T 1 WI and hyperintensed on T 2 WI.On FLAIR images hyperintensed sp ot lesions were revealed in 30cases with onset within one month,hypointensed areas surrounded by hyperintense d ring or semi -ring were observed in another 9cases with onset over 1month,and the above two types of the lesions were demonstrated in 11cases with repeatedly onset during one year.In comparative group multiple spots at gangli on basilar were revealed hypointensed on SET 1 WI and FLAIR sequence,and hyperinte nsed on TSE T 2 WI.Conclusion Differentiating lacunar atroke fro m Virchow -Robin space is available o n FLAIR sequence.
