动脉粥样硬化性脑梗死与低密度脂蛋白亚组分颗粒大小关系的研究

摘    要:目的研究动脉粥样硬化性脑梗死(ACI)发病与低密度脂蛋白(LDL)亚组分颗粒直径大小之间的关系。方法梯度密度超速离心32例ACI患者及32例对照者的血浆LDL,采用2.5%~16.0%聚丙烯酰胺凝胶梯度电泳分析LDL颗粒直径、分布及电泳图形。结果①ACI组三酰甘油(TG),总胆固醇(TC),低密度脂蛋白胆固醇(LDL-C)及载脂蛋白B(ApoB)较对照组明显升高(t=4.03~8.37,P<0.01)。②ACI组LDL颗粒直径犤(24.8±0.7)nm犦较对照组犤(26.0±0.6)nm犦明显变小(t=4.78,P<0.01);两组间A型、B型的分布不同,B型LDL在ACI组分布明显高于对照组(t=6.80,P<0.05)。③ACI组小而密LDL亚组分相对含量犤(50±10)%犦较对照组犤(17±10)%犦明显增多(t=7.76,P<0.001)。④ACI组LDL亚组分颗粒直径与TG(r=-0.47,P<0.01),TC(r=-0.51,P<0.01),LDL-C(r=-0.36,P<0.05),ApoB(r=-0.46,P<0.05)呈负相关。结论ACI组LDL以小颗粒的B型为主,与ACI发生密切相关,TG水平对LDL颗粒直径大小、组成具有调节作用。

Abstract:AIM: To study the correlation of the diameter of small LDL particles with the incidence of atherosclerosic cerebral infarction( ACI) . METHODS: The LDL plasmas were obtained by using density ladder supercentryfugation method in 32 ACI patients and another 32 healthy persons in the control group.The diameter, distribution and electrophoresis areas graphs of the LDL particles were analyzed by using 2.5%- 16.0% non denaturing polyacylamide gradient gels electrophresised method. RESULTS: ① The levels of TG,TC,LDL C, and Apo B in ACI group were significantly high (t=4.03- 8.37,P< 0.01).② The mean diameter of LDL particles in ACI group(24.8± 0.7)nm] were more smaller than the mean diameter in the control group (26.0± 0.6)nm,t=4.78,P< 0.01];the distribution of A and B pattern subfraction between the two groups was different, the distribution of B pattern LDL subfraction in ACI group was more significantly high than that in the control group.(t=6.80,P< 0.05).③ The relative percentage of small and density LDL particles in the ACI group(50± 10)% increased more significantly than that in the control group (t=7.76,P< 0.001). ④ There were negatively correlations between the diameter of LDL subfraction in ACI group with TG(r=- 0.47,P< 0.01),TC(r=- 0.51,P< 0.01),LDL C(r=- 0.36, P< 0.05)and Apo B (r=- 0.46, P< 0.05). CONCLUSION: The dominant LDL subfractions in ACI group were the small and density B pattern LDL that had a closely correlation with the incidence of ACI.It might be one of the dangerous factors of ACI.There was negative correlation between B pattern LDL with the levels of TG that adjusted the diameter and formulation of LDL subfractions.
