椎体骨赘形成机制的研究

摘    要:目的研究椎体骨赘形成的起源和机制。方法实验研究通过建立颈椎间盘退变动物模型,收集动物颈椎标本。临床收集脊髓型颈椎病病人的颈椎间盘及相邻的上下椎体标本。分别在显微镜下观察兔和人颈椎椎体骨赘形成的动态生长过程。结果动物椎体骨赘明显来自于椎体周边关节软骨的增生、钙化和骨化。人颈椎退变颈椎间盘周边软骨终板潮标明显前移,钙化和骨化层增厚,形成突向外侧的椎体边缘的骨赘。结论椎体骨赘来自于周边关节软骨的增生,先形成软骨骨赘,然后通过软骨内钙化和骨化而形成椎体骨赘。

Abstract:Objective To study the mechanism of vertebral osteophyte formation.Method Experimental model of the cervical disc degen-eration in the rabbits was establish ed.The cervical vertebrea of experimentsl rabbits were collected.The c ervical interverbral discs with adj acent bilateral vertebrae which were from22patients with cervical spondylotic myelopathy were cut by anterior cervical surgery.Histological method was applied to study the vertebral osteophyte formation mechanism.Results The osteophyte from the rabbits obviously arisen fromproliferation of t he peripheral articular cartilage.In peripheral cartilage end-plates of human cervical vertebrae of some degenerative cervical discs,its tidem ark ad-vanced,calcified and ossified cartilage thickened obviously,and vertebral osteophyteformation arises f romcontinual calcification and oss ification in peripheral cartilage end-plate.Conclusion The vertebral osteophyte arises fro m proliferating of peripheral artic ular cartilage which undergoes cartilaginous osteophyte,proceed s through an endochondral calcification and ossification,and then chan ges into vertebral osteophyte.
