乳腺癌改良根治术后即刻假体再造乳房的并发症及处理方法

目的探讨保留皮肤的乳腺癌改良根治术后Ⅰ期硅胶假体植入再造乳房的并发症及处理方法。方法 2005年8月～2010年8月64例患者行保留皮肤的乳腺癌改良根治术后Ⅰ期于胸大肌后植入硅胶假体,即时再造乳房,根治术时根据冰冻结果决定是否保留乳头乳晕复合体。结果 64例患者行单侧或双侧保留皮肤的乳腺癌改良根治术+Ⅰ期假体植入再造乳房,1例患者出现包膜挛缩;1例患者出现皮瓣坏死,假体外露,予以皮瓣转移;3例出现皮瓣下积液,予以穿刺抽吸;2例出现假体移位;8例患者因对侧乳房下垂致外观不对称。结论严格掌握手术适应证及术中精细操作可以减少并发症的发生概率。
