摘    要:目的:探讨体力劳动者和脑力劳动者青、中、老年组、男、女性别间餐后脂肪清除规律,建立适合中国人群的脂肪耐量试验方法。方法:选择460名志愿者服脂肪负荷餐,测定空腹及脂肪餐后血浆乳糜浊度(A)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)和总胆固醇(TC)水平,绘制脂质应答曲线和清除时限图。结果:餐后TG随血浆A水平升高和降低,两者呈正相关,体力劳动者组青年妇性,青、中年男性TG在餐后3h达高峰,峰值1.30,2.08,2.42mmol/L.中、老年女性和老年男性TG在餐后5h达高峰,峰值为2.87,293,3.33mmol/L。脑血劳动者中年男、女性、老年男、女性TG峰匀在餐后5小时,升高水平分别为3.15,2.82,4.68,4.57mmol/L,各组餐后8hTG基本恢复至空腹水平。血浆HDL-C与A和TG水平呈负相关,餐后高TG应答导致HDL-C降低。结论:不同劳动强度、年龄和性别间脂肪耐量不同,尤其是脑力劳动者、老年组和女性组对脂肪耐受量较差。脂肪耐量试验基本项目应包括A、TG和HDL-C三项,标本采集时间在空腹和餐后8h有实际意义。

Abstract:Objective To study the characteristics of postprandial fat clearance in healthy farmers and civil workers, and in different sex and age groups. Methods 460 volunteers were selected for study the fat clearance after test meal. Plasma turbidity (A), triglyceride (TG), high dencity lipoprotein cholesterol (HDL C) and total cholesterol (TC) were measured at the fasting and postprandial status.The curves of postprandial lipid response were drawn. Results TG levels were positive correlaed with plasma turbidity after test meal. Plasma TG reached their peaks at postprandial 3 hours in young female, young and middle male farmers. Their peaks were 1 30, 2 08, 2 42 mmol/ L, respectively. Plasma TG reached their peaks at postprandial 5 hours in the middle, old female, and old male farmers, in the middle male and female, old male and female civil workers. Their peaks were 2.87, 2.93, 3.33, 3.15, 2.82, 4.68, 4.57 mmol/L, respectively. They declined to the fasting levels at the 8 hours postprandialy. HDL C decreased after fat meal showed negtive correlation with plasma turbidity and TG levels. Slight decrease in TC levels was noticed after fat meal. Conclusions The fat clearance curves were different in the different age and sex groups. The fat clearance function was declined in the aged subjects,especially in females and civil workers. Our results suggest that the basic items of fat tolerance test should include A, TG and HDL C. The sampling times should be at the fasting state and 8 hours after fat meal in clinic practise.
