360度心理支持促进肺癌手术患者快速康复的效果观察

目的] 探讨360度心理支持对非小细胞肺癌胸腔镜肺叶切除术患者术后快速康复的效果观察。方法]选择2018年1月至2018年9月在我院行胸腔镜肺叶切除术治疗的174例NSCLC肺癌患者作为研究对象，采用随机数字表法分为对照组和观察组各87例。对照组采用常规手术室护理干预，观察组在对照组的基础上给予360度心理支持。观察术前1d和入室后焦虑自评量表（SAS）评分、抑郁自评量表（SDS）评分、收缩压（SBP）、舒张压（DBP）、心率（HR）水平，统计2组护理满意度、拔管天数和住院天数、术前2d及术后3d的睡眠质量、术后并发症的发生率。结果]观察组入室后SAS和SDS评分均低于对照组，差异均有统计学意义（P＜0.05）。入室后观察组患者SBP、DBP、HR均低于对照组，差异均有统计学意义（P＜0.05）。观察组护理满意度显著高于对照组，差异有统计学意义（P＜0.05）。观察组拔管天数和住院天数均低于对照组（P＜0.05），睡眠质量优于对照组、并发症少于对照组。结论]360度心理支持用于非小细胞肺癌患者胸腔镜肺叶切除术手术室护理可改善患者的负性心理，减轻应激状态，提高护理满意度，有利于手术的顺利实施和术后的快速康复。

Effect of 360 Degree Psychological Support on Rapid Rehabilitation of Patients with Lung Cancer Surgery

ABSTRACT: Objective To investigate the effect of 360 degree psychological support on the rapid recovery of lung cancer patients after thoracoscopic lobectomy. Methods: 174 patients with lung cancer who underwent thoracoscopic lobectomy in our hospital from January 2018 to September 2018 were selected as the study subjects. 87 patients in the control group and 87 patients in the observation group were randomly divided into two groups. The control group received routine nursing intervention in the operating room, while the observation group received 360 degrees of psychological support on the basis of the control group. The self-rating anxiety scale (SAS), self-rating depression scale (SDS), systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR) levels were observed 1 day before and after admission. Nursing satisfaction, extubation days and hospitalization days were counted. Sleep quality and incidence of complications 2 days before operation and 3 days after operation were observed. .Results: The SAS and SDS scores of the observation group were lower than those of the control group after admission, and the difference was statistically significant (P < 0.05). The SBP, DBP and HR in the observation group were lower than those in the control group, and the difference was statistically significant (P < 0.05). The nursing satisfaction of the observation group was significantly higher than that of the control group (P < 0.05). The extubation days and hospitalization days of the observation group were lower than those of the control group (P < 0.05), the sleep quality was better than that of the control group, and the complications were less than that of the control group. Conclusion: 360 degree psychological support can improve the negative psychology, reduce stress state and improve nursing satisfaction of patients with lung cancer undergoing thoracoscopic lobectomy, which is conducive to the smooth implementation of surgery and rapid recovery after operation.
