电按摩联合被动运动对脑卒中患者手功能恢复的效果

摘    要:目的探讨电按摩联合被动运动对脑卒中患者早期手功能恢复的效果。方法选择2012年6月—2013年6月收治于江苏省省级机关医院的脑卒中早期手功能障碍患者60例,随机分为观察组和对照组各30例,两组患者均给予手部被动运动训练,观察组在被动运动训练的同时配合手部电按摩。采用Ashworth痉挛评定量表、Brunnstrom偏瘫手功能评价量表、Fugl-Meyer手指运动功能评定量表和FuglMeyer指关节活动度评定表,在入院当天、治疗后10 d和治疗后20 d分别进行疗效评价。结果治疗20d后,观察组在手功能分级和手指运动功能方面明显优于对照组(P0.05);在指关节活动度方面,两组比较差异无统计学意义(P0.05);两组Fugl-Meyer手指运动功能得分均较治疗前有改善(P0.05)。但入院当天和治疗10 d后,两组之间比较差异无统计学意义(P0.05)。结论手部电按摩联合手部被动运动能够有效促进脑卒中患者早期手功能的恢复,对提高脑卒中患者的手部功能和日常生活能力有积极的推动作用。

Abstract:Objective To observe the clinical curative effect of electric massage combined with hand passive movement on hand function rehabilitation in patients with early stroke. Methods A total of 60 early stroke ca- ses with hand dysfunction from one hospital of Jiangsu Province were selected and divided into the experimental group and control group (30 cases in each) from June 2012 to June 2013. The two groups were given hand pas- sive movement training. The experimental group received additional electric massage. Ashworth Spastieity Scale, Brunnstrom Scale, Fugl-Meyer Finger Motor Function Assessment Scale, and Fugl-Meyer referring to the joint activity rating scale were used. The curative effects were compared on the day of admission, 10 d after treatment and 20 d after treatment. Results After treatment for 20 d, finger motor function and hand function classification in the experimental group were improved compared with the control group (P 〈 0.05 ). But there was no significant difference in joint activity between the two groups ( P 〉 0.05 ). Fugl-Meyer score of finger motor function was significantly improved after treatment ( P 〈 0.05 ). But on the day of admission and 10 d af- ter treatment, there was no significant difference between the two groups ( P 〉 0.05 ). Conclusion Electric massage combined with hand passive movement can effectively promote the hand function rehabilitation in early stroke patients.
