导引锻炼联合中药离子导入对神经根型颈椎病的干预效果观察

目的探讨导引锻炼联合中药离子导入对神经根型颈椎病患者的干预效果。方法选取2019年1月至2019年12月华中科技大学同济医学院附属同济医院中医科神经根型颈椎病患者102例,按随机数字表将患者分为导引组、中药组和联合组,每组34例。导引组在中医科专科护士的指导下进行导引锻炼;中药组采用医院自制中药通过中频离子导入治疗仪按摩;联合组先用中药通过中频离子导入治疗仪按摩,按摩结束后再进行导引锻炼。比较3组患者干预前、干预2周后、干预4周后的疼痛视觉模拟评分(VAS)评分以及田中靖久颈椎病症状量表评分,评价不同干预方法对患者的干预效果。结果干预前,VAS评分及田中靖久颈椎病症状量表评分3组差异无统计学意义(P>0.05)。干预2周后,中药组与联合组的VAS评分差异无统计学意义(P>0.05),中药组与联合组的VAS评分均显著低于导引组(P<0.05);在田中靖久颈椎病症状量表得分上,联合组总分显著高于中药组(P<0.05),中药组总分显著高于导引组(P<0.05)。干预4周后,导引组与中药组的VAS评分差异无统计学意义(P>0.05),联合组的VAS评分显著低于导引组与中药组(P<0.05);在田中靖久颈椎病症状量表得分上,联合组总分显著高于中药组(P<0.05),中药组总分显著高于导引组(P<0.05)。结论短期应用导引锻炼联合中药离子导入在缓解神经根型颈椎病患者疼痛方面与单独应用中药离子导入效果相当,但长期应用效果优于单一应用导引锻炼或中药离子导入;导引锻炼联合中药离子导入在改善颈脊神经根受压症状和体征方面效果优于单一应用导引锻炼或中药离子导入。

Nursing effect observation of Daoyin exercise combined with Chinese medicine iontophoresis on patients with cervical spondylotic radiculopathy

Objective To explore the nursing effect of Daoyin exercise combined with Chinese medicine iontophoresis on patients with cervical spondylotic radiculopathy.Methods 102 patients with cervical spondylotic radiculopathy in the Traditional Chinese Medicine Department of Tongji Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from January 2019 to December 2019 were randomly divided into Daoyin exercise group,Chinese medicine group and combined group,with 34 cases in each group.The patients in Daoyin exercise group did Daoyin exercise under the guidance of Chinese medicine specialized nurses.The patients in Chinese medicine group were massaged with hospital-made traditional Chinese medicine through medium frequency iontophoresis therapeutic instrument.And the patients in combined group accepted massage with traditional Chinese medicine first,and then did Daoyin exercise.To evaluate the nursing effects of different intervention methods,the VAS score and Yasuhisa Tanaka symptom scale of cervical spondylosis evaluation were carried out before intervention,2 weeks after intervention and 4 weeks after intervention respectily.Results There was no significant difference in VAS score and Yasuhisa Tanaka symptom scale among the three groups before intervention(P>0.05).There was no significant difference in VAS score between the Chinese medicine group and the combined group after intervention for 2 weeks(P>0.05),and the VAS score were significantly lower in the Chinese medicine group and the combined group than the Daoyin exercise group(P<0.05).The score of Yasuhisa Tanaka symptom scale was significantly higher in the combined group than the Chinese medicine group(P<0.05),and it was significantly higher in Chinese medicine group than the Daoyin exercise group(P<0.05).There was no significant difference in VAS score between the Daoyin exercise group and the Chinese medicine group after intervention for 4 weeks(P>0.05),and the VAS score was significantly lower in the combined group than the Daoyin exercise group and the Chinese medicine group(P<0.05).The score of Yasuhisa Tanaka symptom scale was significantly higher in the combined group than the Chinese medicine group(P<0.05),and it was significantly higher in the Chinese medicine group than Daoyin exercise group(P<0.05).Conclusions The long-term effect of Daoyin exercise combined with Chinese medicine iontophoresis was better than single intervention in relieving pain.The effect of Daoyin exercise combined with Chinese medicine iontophoresis was better than single intervention in improving the symptoms and signs of cervical spinal nerve root compression,and is worthy of clinical application.
