颈椎间盘置换术后监护期间临床护理路径表的设计及应用效果

目的设计颈椎间盘置换术后监护期间临床护理路径表，并探讨其临床应用效果。方法成立临床路径组，设计颈椎间盘置换术后监护期间临床护理路径表并应用于临床。结果应用临床护理路径表后，患者的喉头水肿程度、腹胀程度、术后首次进食时间、气管插管留置时间、血气分析检查次数、ICU停留时间、卧床时间、术后平均住院时间、ICU期间费用等方面均降低（P〈0．01），患者满意度、护士满意度提高（P〈0.01）。结论颈椎间盘置换术后患者早期实施ICU临床护理路径，使术后早期拔气管插管方案有效实施，从而减轻了患者喉头水肿、腹胀程度，使术后首次进食时间提前，降低了ICU停留时间和医疗费用，同时保证了护理质量，提高了患者及护士满意度，是一套兼具品质与成本效益的临床护理路径模式，在临床切实可行，值得推广．

The application and effect of clinical nursing pathway in the stage of post-operative monitoring for patients with cervical artificial disc replacemet.

Objective To explore the methods and effect of the application of elinieal nursing pathway（CNP） in the stage of post - operative monitoring for patients with cervical artificial disc replaeement. Methods The CNP was designed and applied in the stage of post - operative monitoring for patients with cervical artificial disc replacement. Then, the patients＇ recovery index as well as patients＇ and nurses＇ satisfactions were compared before and after this application of CNP. Results The patients＇ larynx edema, flatulence, first time of feeding,indwelling time of tracheal tube, times of blood gas analysis,hospital day in ICU, strict bed - time, average hospital day, medical charge, patients＇ and nurses＇ satisfaction before and after the application of CNP （P 〈 0.01 ）. Conclusion The application of CNP in the early postoperative stage for patients with cervical artificial disc replacement can promote the patients＇ recovery,relieve their symptoms,reduce medical charge and improve both the patients ＇ and nurses ＇ satisfaction, which is proved to be a feasible means with high quality and costbenefit,and deserves to be spread in clinical practice.
