| 摘 要： | 目的:探讨结节性甲状腺肿伴乳头状增生与甲状腺乳头状癌的组织形态学特点、两者间的关系及鉴别诊断。方法:对469例结节性甲状腺肿及39例甲状腺乳头状癌进行临床病理分析。结果:469例中34例(7.25%)伴单纯性乳头状增生,17例(3.62%)伴非典型乳头状增生,7例(1.49%)伴癌变。39例乳头状癌中12例(30.77%)伴结节性甲状腺肿,2例(5.13%)伴桥本甲状腺炎。结论:结节性甲状腺肿是甲状腺癌的癌前疾病,对伴有非典型性增生的乳头状病变应加强随访。

| Abstract: | Objective To explore the histomorphologic features,differential diagnosis and relationship between nodular goiter with papillary hyperplasia and papillary thyroid carcinomas.Methods Four hundred and sixty-nine cases of nodular goiter and thirty-nine cases of papillary thyroid carcinomas were clinicalpathological analyzed.Results Nodular goiter was complicated with papillary hyperplasia in 7.25%,with atypical papillary hyperplasia in 3.62%,with carcinomatous change in 1.49%.Papillary thyroid carcinoma was with nodular goiter in 30.77%,with Hashimoto's thyroiditis in 5.13%.Conclusion Nodular goiter is the precancerous disease of thyroid carcinoma.Goiter cases with atypical papillary hyperplasia should be closely followed up. |
