针刺配合GnRH-a治疗卵巢巧克力囊肿剥除术患者的疗效观察

目的观察针刺配合促性腺激素释放激素激动剂(GnRH-a)对卵巢巧克力囊肿(OEC)剥除术患者的治疗效果。方法选取2015年6月至2018年10月张家港市中医医院诊治的86例行OEC剥除术的患者作为研究对象。通过随机分组的方式将患者分为观察组(n=43)与对照组(n=43)。对照组术后接受常规治疗,观察组在对照组基础上采用针刺配合GnRH-a治疗,比较两组治疗前后的内分泌功能、卵巢储备功能、月经情况、囊肿复发率及妊娠结局。结果治疗后,观察组E 2、FSH、LH等激素水平低于对照组,差异具有统计学意义(P<0.05);观察组基础状态卵泡数、卵巢体积优于对照组,差异具有统计学意义(P<0.05);观察组月经期、痛经VAS评分低于对照组,差异具有统计学意义(P<0.05);观察组囊肿复发率和妊娠成功率优于对照组,差异具有统计学意义(P<0.05)。结论针刺配合GnRH-a可以有效调节OEC患者的血清性激素水平,改善患者的卵巢储备功能,降低患者术后复发率,提高妊娠成功率,值得进行临床推广。

Clinical effect of acupuncture combined with GnRH-a on the patients with the removal of ovarian endometriosis cyst

Objective To observe the therapeutic effect of acupuncture combined with gonadotropin-releasing hormone agonist GnRH-a on patients with ovarian endometriosis cyst(OEC).Methods A total of 86 patients underwent ovary chocolate cyst removal admitted to Zhangjiagang Hospital of Traditional Chinese Medicine were selected as the study subjects,and were randomly divided into observation group(n=43)and control group(n=43).The control group received conventional treatment after surgery,while the observation group was combined with acupuncture and GnRH-a.The differences in endocrine function,ovarian reserve function,menstrual status,cyst recurrence rate and pregnancy outcome between the two groups before and after treatment was compared.Results After treatment,the levels of E 2,FSH,LH and other hormones in the observation group were significantly lower than those in the control group(P<0.05).The number of follicles and ovarian volume in the observation group were significantly better than those in the control group(P<0.05).VAS scores of menstruation and dysmenorrhea in the observation group were significantly lower than those in the control group(P<0.05).The recurrence rate of cysts and pregnancy success rate in the observation group were significantly better than those in the control group(P<0.05).Conclusions Acupuncture combined with GnRH-a can effectively regulate the serum sex hormone level of OEC patients,improve the ovarian reserve function,reduce postoperative recurrence rate,and increase the success rate of pregnancy,which is worthy of clinical promotion.
