左布比卡因配合真空负压水动气动气泡按摩联合盐酸达泊西汀治疗早泄的临床观察

目的探讨左布比卡因配合真空负压水动气动气泡按摩联合盐酸达泊西汀在早泄治疗中的效果和安全性。方法选取2017年9月至2018年12月合肥市第二人民医院门诊就诊的77例早泄患者作为研究对象。按照就诊时间将其随机分为试验组和对照组。试验组39例患者采用左布比卡因配合男性性功能康复治疗仪,每周3~5次,每次30分钟,持续3个月,同时加服30mg盐酸达泊西汀(必利劲),性生活前1小时口服。对照组38例患者仅采用30mg盐酸达泊西汀治疗,性生活前1小时口服,治疗周期为3个月。分别记录两组患者治疗前后所需观察指标,如射精潜伏期(IELT)、早泄诊断工具(PEDT)评分、配偶性生活满意度评分、合并勃起功能障碍(erectile dysfunction,ED)者国际勃起功能问卷-5(IIEF-5)评分、PE疗效评判及合并ED者ED疗效评判。结果试验组39例患者治疗后,PE有效11例,治愈22例,无效6例,PE总有效率84.6%,合并ED者勃起功能改善率75.0%;对照组38例患者治疗后,PE有效8例,治愈16例,无效14例,总有效率63.2%,合并ED者勃起功能改善率23.1%。两组患者治疗后在IELT、PEDT、配偶性生活满意度、合并ED者IIEF-5评分方面均比治疗前有所改善,其差异均具有统计学意义(均P<0.05)。治疗后,试验组患者在IELT、PEDT、配偶性生活满意度、合并ED者IIEF-5评分方面均优于对照组患者,其差异均具有统计学意义(均P<0.05)。结论左布比卡因配合真空负压水动气动气泡按摩联合盐酸达泊西汀治疗PE疗效显著,安全性良好,且能够明显改善合并ED患者勃起功能,值得临床推广应用。

Clinical observation of levobupivacaine and dapoxetine hydrochloride combined with vacuum negative pressure hydrodynamic/pneumatic bubble massage for premature ejaculation

Objective To investigate the efficacy and safety of levobupivacaine and dapoxetine hydrochloride combined with vacuum negative pressure hydrodynamic/pneumatic bubble massage in the treatment of premature ejaculation(PE). Methods 77 patients with PE in Hefei Second People′s Hospital from September 2017 to February 2018 were randomly divided into experimental group(n=39) and control group(n=38) according to the time of treatment. The experimental group used levobupivacaine combined with male sexual function rehabilitation therapeutic device, 3-5 times a week, 30 minutes each time, for 3 months, plus 30 mg of dapoxetine hydrochloride(Bilixin), orally taken an hour before sexual life. The control group was only treated with 30 mg of dapoxetine, orally an hour before sexual life. Both groups were treated for 3 months. The indicators that should be observed before and after treatment, including intra-vaginal ejaculation latency time(IELT), premature ejaculation diagnostic tool(PEDT) score, spouse′s life satisfaction score, international erectile function questionnaire-5(IIEF-5) score of patients combined erectile dysfunction(ED), PE efficacy evaluation and efficacy evaluation of patients combined with ED. Results After treatment, there were 11 cases effective in PE, 22 cases cured and 6 cases ineffective in the experimental group, the total effective rate of PE being 84.6%, and the improvement rate of erectile function in ED patients being 75.0%. In control group, there were 8 cases effective in PE, 16 cases cured and 14 cases ineffective, the total effective rate being 63.2% and the improvement rate of erectile function in ED patients being 23.1%. After treatment, the IELT, PEDT, spouse′s sexual life satisfaction, and IIEF-5 scores of ED patients were improved compared with those before treatment, with statistically significant differences(all P<0.05). In the experimental group, the IELT, PEDT, spouse′s sexual life satisfaction, and IIEF-5 scores of ED patients were better than those in the control group, with statistically significant differences(all P<0.05). Conclusions Levobupivacaine and dapoxetine combined with vacuum negative pressure hydrodynamic/pneumatic bubble massage for the treatment of PE is effective, safe, and can significantly improve the erectile function of patients with ED, which is worth promoting.
