梅毒患者的颅脑MRI表现研究

摘    要:目的:探讨梅毒患者颅脑核磁共振检查(MRI)表现,以期更好治疗该疾病。方法:回顾性分析2010年3月至2013年1月34例梅毒患者,选取同时期正常患者33例为对照组,对两组均进行实验室检查,观察相关指标变化情况,同时对梅毒患者进行MRI检查,观察影像学变化。结果:对照组血清RPR、TPPA和脑脊液RPR、TPPA均为阴性,而梅毒组血清RPR、TPPA和脑脊液RPR、TPPA均为阳性,且蛋白和白细胞计数上对照组明显低于梅毒组,以上指标两组比较差异显著(P0.05)。MRI表现上有23例为脑膜血管型,11例为树胶肿型,表现为多发病灶,大小不等,T1W1上低信号或混杂信号,T2W1上高信号,T1W1增强表现为不规则强化。结论:梅毒患者以实验室检查为主,MRI对梅毒无特异性表现,但能显示其病变范围,对早期治疗有一定意义。

Abstract:Objectives:To investigate the application of brain MRI on patients with syphilis for better treatment.Methods:A retrospective analysis was conducted on the data of 34 syphilis patients from March 2010 to January 2013,and another 34 healthy subjects were in the same period were chosen as the control group.Both groups received laboratory tests to observe the changes in relevant indexes,while patients with syphilis received additionally MRI examination to observe imaging changes.Results:The serum RPR,TPPA and CSF RPR,TPPA were negative in control group,but positive in the syphilis group;the protein and white blood cell count in the control group was significantly lower than that in the syphilis group,all with significant difference (P < 0.05).According the MRI findings,there were 23 cases of meningeal vascular type and 11 cases of gum swelling type,with characteristics like multiple lesions and varying sizes,T1W1 low signal or mixed signal,T2W1 high signal,and irregular enhanced T1W1.Conclusion:Laboratory test should be applied on syphilis patients.MRI can show the lesion size,which is helpful for early treatment.
