心脏骤停患者现场心肺复苏成功影响因素分析

摘    要:目的探讨影响现场心肺复苏成功的相关因素。方法抽查2009年复苏成功率较高的11名医生的复苏成功病例80份(复苏成功组)及复苏不成功病例87份(复苏无效组)(共167例)。分析患者因素,患者家属因素以及急救中心急救反应速度对复苏成功的影响。结果①两组发病年龄具有相同的分布特点(P〉0.05),均呈偏态分布,发病年龄主要集中在70岁以上,平均78岁。女性发病年龄略高于男性。②复苏成功组患者中心电图表现为无脉搏心电活动(PEA)者占45%,显著高于复苏无效组(17%,P〈0.05)。③复苏成功组发现患者意识丧失,呼之不应后平均10.2min发出呼救,呼救时间显著短于复苏无效组(平均为21.7min,P〈0.05)。④复苏成功组现场施行CPR的占5%,复苏无效组2.3%(P〉0.05)。⑤从接到调度指令至到达现场施救的时间两组都平均在7min左右(分别为7.25min和7.83min,P〉0.05)。结论患者家属现场自救意识和能力普遍欠缺。早发现,及时启动急救反应系统是复苏成功的关键。

Abstract:Objective To investigate the influencing factors associated with the successful eardiopulmonary resuscitation (CPR) of cardiac arrest (CA) victims. Methods The clinical data of 167 CA victims with similar age distribution treated by 11 senior EMS doctors with high successful rates, of which 80 attained restoration of spontaneous circulation (ROSC) (successful group) and 87 failed to regain spontaneous circulation (ineffective group). The relevant factors, such as the conditions of the victims, behaviors of the first responders (mainly family members), and the responding speed of the ambulances were compared. Results (1)The ages of these 2 groups showed a skewed distribution (P〉0.05) , with the main age of onset being over 70, and the CA onset age of the female patients was slightly more advanced than that of the males (80 vs. 76). (2)The prevalence of pulseless electronic action (PEA)in ECG of the successful group was 45%, significantly higher than that of the ineffective group (17%,P〈0.05). (3)The first responders of the successful group sent a call for help 10.2 minutes after they discovered that the patients were comatose on average, a time significantly shorter than that of the ineffective group (21.7 minute, P〈0.05). (4)The bystander CPR rate of the successful group was 5%, and that of the ineffective group was 2.3% (P〉0.05). (5) The time between receiving the dispatching command and the arrival of ambulance at the site was (7.25±0.40)minutes in the success group, not significantly different from that of the ineffective group (7.25 ±0.40)minutes,P〉0.05]. Conclusion The consciousness and ability of self-aid of the family members of CA patients are low in general. Early discovery and activation of the emergency response system are the keys of successful resuscitation.
