阴囊皮脂腺癌1例

患者男,72岁,双侧阴囊反复红斑伴痒2年余,新发肿块2个月。皮肤科情况:双侧阴囊见局限性境界清楚的浸润性暗红斑,表面有浅糜烂、渗液;右侧阴囊红斑皮损上见一处约1.5 cm×1.0 cm×0.3 cm大小的暗红色肿块,大致呈半球状,触之质稍硬,表面不光滑,活动度尚可。皮损组织病理示:真皮内肿瘤细胞呈巢团状分布,胞浆丰富、呈嗜酸性,核仁明显,可见核分裂象,中间有纤维间隔,于皮肤附属器周围浸润,周围表皮内可见Paget病样播散的肿瘤细胞。免疫组织化学:EMA、CKpan、CKL、CK7、AR、CD15、CA153阳性,CK14、CK17、CK20、CEA、Mamaglobin、GCDFP-15、CD34、D2-40阴性,Ki67增殖指数为30%。诊断:阴囊皮脂腺癌。

A Case of Scrotal Sebaceous Carcinoma

A 72-year-old male presented bilateral scrotum recurrent erythema with pruritus for more than 2 years,new mass for 2 months.Physical examination revealed infiltrating dark erythema with localized boundary,superficial erosion and exudation on the scrotum.On the erythema lesions of right scrotal had a dark red mass,with non-smooth surface,slightly stiffly touched and good activity in shape of roughly hemisphere and size of 1.5 cm×1.0 cm×0.3 cm.Histopathologic examination showed the tumour cells with rich in acidophilic cytoplasm,obvious nucleolus,mitotic phase in the dermis and fiber spaced in the middle were arranged in nests,infiltrated around the skin appendage and spread like Paget′s disease in the surrounding epidermis.Immunohistochemistry study showed positive to EMA,CKpan,CKL,CK7,AR,CD15,CA153,Ki-67 and negative to CK14,CK17,CK20,CEA,Mamaglobin,GCDFP-15,CD34,D2-40.The scrotal sebaceous carcinoma was diagnosed.
