摘    要:目的通过有限元法对内收型股骨颈骨折的不同内固定方法对比评价,寻找合理的固定方式,为临床应用提供理论依据.方法以内收型股骨颈骨折为研究对象建立模型,模拟单腿站立时的受载情况,在固定角度一定的情况下分别进行有限元计算.结果两钉竖放固定时的抗弯抗张能力和稳定性较好,但抗扭性较差,三钉固定时按倒△方式固定时的名指标均比较好,单钉固定时综合效果并不最差,而且临床操作简单,手术时间短.结论固定效果与螺钉的数量和组合方式有着直接的联系,根据病人的实际情况合理选择固定方式对于股骨颈骨折的愈合具有重要的作用.

Abstract:Purpose the different internal fixation means of the adductive femoral neck fracture were compared and appraised by the ways of the finite element method to find out the rational fixation manner and offer the theoretical foundation for clinical practice.Method the model was set up according to the adductive femoral neck fracture as the research object, and the method of FEA (Finite element analysis) was used to compare the biomechanical feature of the different fixation manners in the same fixation angle. The mode of standing on one leg was simulated as the load situation. The results: the fixation manner of two screws in vertical group is better in bending and stretching resistance abilities, better stability, but worse torsion resistance. The fixation manner of three screws in negative triangle group has good results in all the parameters. The comprehensive result of using the single screw is not the worst. Moreover it is very simple in clinic, so it save operation time. Conclusion the fixation effect has direct contacts with the quantity of the screw and mode of combination. Selecting the rational fixation manner according to the patients' situation is more important to the femoral neck fracture's concrescence.
