肺表面活性物质治疗晚期早产儿呼吸窘迫综合征疗效观察

摘    要:目的探讨肺表面活性物质治疗晚期早产儿呼吸窘迫综合征(RDS)的疗效。方法 74例晚期早产儿RDS分为2组,对照组36例给予机械通气、预防感染和脑损伤、肠内和肠外营养支持等治疗,观察组38例在此基础上给予肺表面活性物质气管内滴入治疗;2组患者分别于治疗前及治疗6、24、48 h进行血气分析,并判定疗效。结果 2组临床疗效比较差异无统计学意义(P>0.05);2组患儿治疗前动脉血气分析指标比较差异均无统计学意义(P>0.05),2组患儿治疗后动脉血气分析指标均有显著改善(P<0.05,P<0.01),观察组改善更显著(P<0.05)。结论肺表面活性物质治疗晚期早产儿RDS可以有效改善低氧血症。

Abstract:Objective To explore the curative effect of pulmonary surfactant in treatment of late preterm infants with respiratory distress syndrome(RDS).Methods Seventy-four late preterm infants with RDS were divided into two groups.Thirty-six patients in control group were treated with mechanical ventilation,infection prevention,prevention of brain injury,enteral and parenteral nutrition;based on this,thirty-eight patients in observation group were treated with pulmonary surfactant through intratracheal instillation.The blood gas analysis was performed before treatment and six,twenty-four,forty-eight hours after treatment in two groups;and the curative effect was evaluated.Results There was no significant difference in the clinical effect between two groups(P>0.05).Before treatment,there was no significant difference in the indexes of arterial blood gas analysis between two groups(P>0.05).After treatment,the index of arterial blood gas analysis improved significantly in two groups(P<0.05,P<0.01),but the improvement was more in observation group(P<0.05).Conclusion Pulmonary surfactant can effectively improve hyoxemia in late preterm infants with RDS.
