1型糖尿病患者血清可溶性B7-H4的表达及临床意义

摘    要:目的探讨1型糖尿病(T1DM)患者血清可溶性B7-H4水平变化及临床意义。方法检测39例T1DM患者(T1DM组)和20名健康体检者(对照组)血清B7-H4分子表达情况,并分析其表达水平与糖化血红蛋白(HbAlc)、胰岛素(In)、微量白蛋白尿(MAU)、谷氨酸脱羧酶抗体(GADA)、胰岛细胞抗体(ICA)和胰岛素抗体(IAA)的关系。结果 T1DM组血清可溶性B7-H4水平显著低于对照组,HbAlc及MAU水平、GADA、ICA及IAA阳性率均显著高于对照组(P<0.05);但2组血清In水平比较差异无统计学意义(P>0.05)。B7-H4表达水平与MAU呈负相关(r=-0.27,P<0.01)。结论 B7-H4表达下降可能是T1DM及糖尿病肾病发病机制之一。

Abstract:Objective To explore the clinical significance of serum soluble B7-H4 level change in patients with type 1 diabetes mellitus(T1DM). Methods Expressions of serum B7-H4 in thirty-nine patients with T1DM(T1DM group) and twenty healthy peoples(control group) were detected.The relationships of B7-H4 level with glycosylated hemoglobin(HbAlc),insulin(In),microalbuminuria(MAU),glutamic acid decarboxylase antibody(GADA),insular cellular antibody(ICA)and insulin autoantibody(IAA) were analyzed. Results Compared with control group,the level of serum soluble B7-H4 was significantly lower,but the levels of HbAlc,MAU,and the positive rates of GADA,ICA,IAA were significantly higher in T1DM group(P<0.05).There was no statistical difference in the level of In between two groups(P>0.05).B7-H4 level showed negative correlation with MAU level(r=-0.27,P<0.01). Conclusion B7-H4 expression decline may be one of the pathogenesy of T1MD and diabetic nephropathy.
