腹腔镜联合胆道镜治疗胆道结石疗效观察

目的探讨腹腔镜联合胆道镜治疗胆道结石的临床疗效。方法将88例胆道结石患者分为2组,观察组48例施行腹腔镜联合胆道镜手术,对照组40例施行开腹手术,比较2组患者的临床疗效。结果 2组患者手术时间比较差异无统计学意义(P>0.05);观察组患者术中出血量少于对照组(P<0.05),术后排气时间和住院时间显著短于对照组(P<0.05),切口感染率显著低于对照组(P<0.05)。观察组无胆道狭窄及结石复发患者,对照组术后出现胆总管结石复发4例,行再次手术,无胆道狭窄患者。结论腹腔镜联合胆道镜治疗胆道结石疗效确切,有手术安全、创伤小、恢复快、住院时间短、并发症少等优点。

Effect of laparoscope combined with choledochoscope on bile duct stones

Objective To explore the clinical effect of laparoscope combined with choledochoscope in the treatment of bile duct stones.Methods Eighty-eight patients with bile duct stones were randomly divided into two groups,forty-eight patients in the observation group were treated with laparoscope and choledochoscope surgery,forty patients in the control group were treated with open surgery.The clinical effect was compared between two groups.Results There was no statistically significant difference in operation time between two groups(P<0.05).Compared with the control group,the intraoperative blood loss was less(P<0.05),postoperative intestinal function recovery time and hospitalization time was significantly shorter(P<0.05),and the incision infection rate was significantly lower in the observation(P<0.05).There was no cases of biliary tract stricture and stone recurrence in observation.There were four cases of common duct stones recurrence after operation in control group,and the patients were treated with the second operation,no patients with biliary tract stricture.Conclusion Laparoscope combined with choledochoscope for the treatment of bile duct stones have preonunced effect,safe operation,less trauma,quicker recovery,shorter hospitalization time,and less complications.
