不可复性颞下颌关节盘前移位术前与术后可视性个体化建模

摘    要:
目的研究构建正常和不可复性颞下颌关节盘前移位及在两种不同治疗方式下张、闭口位的颞下颌关节系统的个体化三维可视模型,为颞下颌关节盘前移位的生物力学致病机理研究及其临床治疗方法的评价提供依据。方法选择正常志愿者和单侧不可复性颞下颌关节盘前移位患者各1名,采用CT和MRI两种扫描方式,按照闭口位与最大张口位的姿势进行扫描,应用Mimics和Geomagic软件平台,建立正常的与病理的颞下颌关节三维几何模型。同时,患者行颞下颌关节腔内注射透明质酸治疗后,模拟关节盘复位手术后分别进行闭口位与最大张口位姿势的扫描与重建。结果建立了正常的、不可复性颞下颌关节盘前移位术前的、行透明质酸关节腔内注射后的、模拟关节盘复位的4组个体化三维可视模型。结论基于CT、MRI扫描和Mimics、Geomagic三维重建能够实现不可复性颞下颌关节盘前移位行透明质酸关节腔内注射和模拟关节盘复位的个体化三维可视建模,对评价颞下颌关节紊乱病的临床治疗效果具有一定指导意义,并为颞下颌关节盘前移位的生物力学致病机理研究提供了形态学基础。
