三种血清肿瘤标志物检测在原发性肝癌诊断中的意义

目的探讨恶性肿瘤相关物质(TSGF)与甲胎蛋白(AFP)、铁蛋白(SF)并用诊断原发性肝癌的临床意义.方法用化学显色法和放射免疫分析法检测40例健康人、47例急性肝炎、41例肝硬化和38例原发性肝癌血清中TSGF、AFP、SF的含量.结果 TSGF与AFP、SF联合检测的敏感性达98.4%,特异性达99.0%,阳性预测值为100%,TSGF、AFP和 SF的阳性似然比依次为8.1、10.7和5.1,阴性似然比依次为0.21、0.22和0.28.结论 TSGF与AFP、SF指标联用,可进一步提高原发性肝癌诊断的敏感性和准确性,联合检测对原发性肝癌的诊断具有十分重要的意义.

Significance of detection of 3 serum tumor markers in the diagnosis of primary hepatocellular carcinoma

Objective To study a method of diagnosis of primary hepatocellular carcinoma (PHC) using tumor supplied group of factors (TSGF) combining with AFP and ferritin(SF) as the tumor markers. Methods Biochemical colorimetry and RIA were used to analyze the serum contents of TSGF, AFP and SF in 40 healthy people,47 acute viral hepatitis patients, 41 cirrhosis of liver patients and 38 PHC patients. Results For the TSGF combining with AFP and SF, the sensitivity was 98.4%, the specificity was 99.0% and the positive predictive value was 100% .Their positive likelihood ratios were 8.1,10.7 and 5.1 respectively and the negative likelihood ratios were 0.21,0.22 and 0.28 respectively. Conclusion This combining method can improve the sensitivity and accuracy of diagnosis primary hepatocellular carcinoma and shows a very important function for diagnosing primary hepatocellular carcinoma.
