妊娠合并卵巢肿瘤蒂扭转34例临床分析

目的探讨妊娠合并卵巢肿瘤蒂扭转的临床诊断和治疗方法。方法回顾性分析34例妊娠合并卵巢肿瘤蒂扭转患者临床资料。结果 34例妊娠合并卵巢肿瘤蒂扭转患者中误诊为急性阑尾炎2例、卵巢肿瘤破裂3例;6例保守治疗,28例手术治疗,其中9例行高位结扎卵巢动、静脉后给予卵巢肿瘤复位加肿瘤剥除术,19例行患侧附件切除术;获得健康新生儿28例(足月产22例,早产6例),流产6例。结论妊娠合并卵巢肿瘤蒂扭转是妇产科急症,及时诊断及积极手术治疗母婴预后良好。

Diagnosis and therapy of ovarian tumors torsion in 34 pregnant patients

Objective To explore the clinical diagnosis and treatment of ovarian tumors torsion in pregnancy.Methods The clinical data of 34 patients with ovarian tumors torsion in pregnancy were retrospectively analyzed.Results In 34 patients,5 cases were misdiagnosed(2 cases were misdiagnosed as acute appendicitis and 3 cases were misdiagnosed as ovarian tumors rupture).Six cases received conservative treatment,and the other 28 cases were performed operation,in which 9 cases were performed high ligation of the suspensory ligament of ovary followed by reposition plus excision of ovary benign tumors,and the other 19 cases were performed adnexectomy.Twenty-eight healthy neonates were delivered(22 babies of term birth and 6 of premature birth) and miscarriage occurred in 6 cases.Conclusion Ovarian tumors torsion in pregnancy is mobstetrical emergency.To make a prompt diagnosis and perform correct treatment can achieve a good result for mother and infant.
