Q开关532nm激光和强脉冲光治疗面部雀斑的疗效和安全性比较

目的:探讨Q开关Nd:YAG 532nm激光和I PL治疗面部雀斑的临床疗效和安全性。方法:将200例面部雀斑患者随机分为QS532组100例,I PL组100例,分别采用QS532激光治疗仪治疗1次,I PL治疗仪治疗2次,比较两者的疗效和安全性。结果:QS532组患者治疗后第4周,皮损平均消退达82.25%;I PL组治疗1次后第4周皮损平均消退达60.72%,QS532组治疗效果优于I PL组;QS532组治疗后第12周,患者皮损平均消退达82.54%,I PL组治疗2次后第12周皮损平均消退达77.80%,QS532组治疗效果更明显。患者对治疗的满意度评价,QS532组非常满意率和满意率分别为71%和29%,无轻度满意和不满意的患者;IPL组非常满意率和满意率分别为76%和20%,轻度满意率为4%,无不满意患者。两组满意度比较无统计学差异。QS532组,有26例患者于术后4周内患处出现色素加深,占总例数的26%,均于6个月内消退;I PL组,无炎症后色素沉着的病例出现。两组均未出现瘢痕、色素减退、永久性色素沉着等不良反应。结论:QS532激光和IPL均可很好地治疗雀斑,QS532激光治疗效果优于IPL,I PL的安全性优于QS532激光,我们应根据患者的不同情况,选择最合适的治疗方式,两者可以联合应用,取长补短。

Comparison of efficacy and safety between Q-switched 532 laser and intense pulsed light for the treatment of facial freckles

Objective To discuss the efficacy and safety of Q-switched Nd:YAG 532nm laser and intense pulsed light(IPL) for the treatment of facial freckles.Methods 200 cases of facial freckles were randomly grouped into 100 cases of group QS532 and 100 cases of group IPL.Patients of group QS532 were treated by QS532 laser once,while patients of group IPL were treated by IPL twice.Compare the efficacy and safety of the two groups.Results Both of the groups of QS532 laser and IPL acquired obvious improvement,the improvement rate of group QS532 was 82.25% 4 weeks after treatment,while it was 60.72% after treated by IPL the first time 4 weeks later,the efficacy of group QS532 was better than that of group IPL;the improvement rate of group QS532 was 82.54%,which was also better than 77.80% of group IPL treated by IPL the second time 12 weeks later.According to the satisfaction rate,the rates of very satisfied and satisfied of group QS532 were 71% and 29%,there were no patients slightly satisfied and dissatisfied,the rates of very satisfied and satisfied of group IPL were 76% and 22%,the rate of slightly satisfied was 2% and no patients dissatisfied.There was no significant statistical difference of the rates of very satisfied and satisfied between the two groups.There were 26 cases of patients out of 100 cases of group QS532 developed PIH(26%),which subsided in 6 months while no one developed it after IPL therapy.Conclusions Both of QS532 laser and IPL can treat facial freckles well,the efficacy of QS532 laser is better than that of IPL,while the safety of IPL is better than that of QS532 laser.In clinical,the most appropriate treatment should be chosen by different patient and different situation,the two kinds of treatment can be combined application and complement each other.
