体外循环预充贺斯对预充液胶体渗透压的影响

目的观察体外循环中预充6％经乙基淀粉对预充液胶体渗透压的影响。方法20例（实验组）心内直视手术患者体外循环预充液总量的3／4采用6％经乙基淀粉（30mL／kg），另1／4采用平衡液，4％碳酸氢钠（50-100mL）和20％甘露醇（50-100mL），与20例（对照组）体外循环总量的3／4采用平衡盐，另1／4采用血浆。4％碳酸氢钠（50-100mL）和20％甘露醇（50-100mL）的患者比较，观察体外循环中6％经乙基淀粉对预充液的胶体渗透压的影响。结果在基本相同的条件下，实验组的胶体渗透压比对照组的胶体渗透压明显增高，两组相比有明显的差异性（P〈0．01）。结论体外循环中大剂量预充贺斯可明显提高转流过程中的胶体渗透压。

Effect of extracorporeal circulation priming HS on colloid osmotic pressure

[Objective] To observe the effect of 6% hydroxyethelstarch (HES) for colloid osmosis pressure(COP) during cardiopulmony bypass (CPB). [Methods] In test group (20 cases), the 3/4 volume of total volume was HES (30 mL/kg), the other 1/4 volume contain 4% NaHCO3 (50~100 mL), 20% mannitol (50~100), in control group (20 cases), the 3/4 volume of total volume was crystal, the other 1/4 volume contain 4% NaHCO3 (50~100 mL), 20% mannitol (50~100 mL) and blood plasma (200~250 mL), to observe the effect of HES for COP during CPB. [Results] In the same condition, the average COP in test group was much higher than contain group (P<0.01). [Conclusion] Large dose HES could rise COP during CPB.
