摘    要:目的了解秦皇岛地区健康中老年群体腰椎骨密度(BMD)水平及骨质疏松状况,探讨定量CT(QCT)测量在诊断该地区中老年骨质疏松中的应用价值。方法选取2016年1月至2018年4月在秦皇岛市骨科医院进行体检的中老年人1 299例,应用采用美国GE公司生产的全身螺旋CT机配套体模及应用软件进行第2、3、4腰椎骨密度定量测量,比较不同性别、不同年龄组BMD水平及骨质疏松发生情况。结果在研究纳入的全部1 299人中,骨质疏松的比例平均为49.0%,其中男性为33.0%,低于女性的57.0%,差异具有统计学意义(χ~2=66.636,P0.001)。男性和女性BMD在45岁之前一直维持在较高水平,40~44岁年龄组BMD水平分别为(153.03±35.40) mg/cm~3和(165.81±31.84) mg/cm~3。在45~49岁年龄组,男性和女性BMD峰值开始出现第一次下降,在55~59岁年龄组开始出现第二次下降,女性在60岁之后BMD水平下降幅度要大于男性。结论本研究探索了秦皇岛市1 299例中老年人不同性别、不同年龄组的BMD水平以及骨质疏松发生情况,对秦皇岛地区中老年人群QCT骨密度测量和骨质疏松诊断有重要参考价值。

Abstract:Objective To understand the level of lumbar spine bone mineral density (BMD) and osteoporosis in healthy middle-aged and elderly populations in Qinhuangdao and explore the value of quantitative computed tomography (QCT) measurement in the diagnosis of osteoporosis among the middle-aged and elderly in this area. Methods 1299 cases of middle-aged and elderly individuals who took physical examines in Orthopaedic Hospital of Qinhuangdao from January 2016 to April 2018 were selected. Quantitative measurement of the second, third and fourth lumbar spine BMD was performed by using the phantom and application software of the spiral CT produced by the American GE company. The BMD levels and occurrence of osteoporosis in different genders and age groups were compared. Results Among all the 1299 people included in this survey, the proportion of osteoporosis was 49.0% on average, of which 33.0% were men, which was lower than 57.0% of women. The difference was statistically significant (?2=66.636, P<0.001). The BMD of males and females has been maintained at a high level before the age of 45. In the 40-44 age group, the BMD of males and females were153.03±35.40 mg/cm3 and 165.81±31.84 mg/cm3 respectively. In the 45-49 age group, the peak BMD of men and women began to decline for the first time, and the 55-59 age group began to decline for the second time. The decline rate of BMD level in women after 60 years of age is greater than that in men. Conclusion BMD levels and osteoporosis of 1299 middle-aged and elderly people from Qinhuangdao in different genders and age groups which were explored in this study has important reference value for QCT bone mineral density measurement and osteoporosis diagnosis in the middle-aged and elderly population in Qinhuangdao.
