对儿童幽门螺杆菌粪便抗原检测方法的评价

目的探讨适合诊断儿童幽门螺杆菌(Hp)感染的检测方法.方法收集93例儿童粪便,用酶免疫反应进行Hp粪便抗原(HpSA)试验,以13C-尿素呼气实验(13C-UBT)和(或)胃镜检查作为诊断Hp感染的金标准.结果金标准诊断阳性44例,阴性49例."金标准"阳性的44例中HpSA检测阳性40例,"金标准"阴性4例;阴性的49例中HpSA检测阴性43例,阳性6例.HpSA检测诊断儿童Hp感染的敏感性为90.91%(40/44),特异性为87.76%(43/49),阳性预测值为86.96%(40/46),阴性预测值为91.49%(43/47),诊断准确性为89.25%(83/93).结论HpSA实验是一种简便、准确的诊断儿童Hp感染的检测方法,特别适用于儿童.

Evaluatino of children stool antigen test in the diagnosis of Helicobacter pylori infection

Objective The aim of this study was to find out the effective diagnostic measurement for the infection of Helicobacter pylori(Hp)in children. Methods 93children stool samples were tested with Helicobacter pylori stool antigen assay(HpSA), and 13C-Urea breath test(13C-UBT), which was regarded as the gold standard for the diagnosis of Helicobacter pylori infection. Results With the gold standard 13C-UBT, 44 cases had positive results and the other 49 had negative result. With HpSA, 40 cases out of the 44 positive cases were diagnosed with the same positive results,4 cases with negative results;of the 49 negative ones, HpSA found 43 negative but with infection of Hp and the other 6 were positive. HpSA as the diagnostic measurement for Hp infection had the sensitivity 90.91%(40/44), and the specificity 87.76%(43/49). It's positive predictive value was 86.96%(40/46), and the negative predictive value was 91.49%(43/47). It's accuracy was 89.25%(83/93). Conclusions HpSA is a simple and accurate diagnostic method for the infection of Helicobacter pylori.It is especially useful in children.
