放疗后下肢水肿患者股动脉弹性功能改变及其意义的研究

摘    要:目的应用血管回声跟踪技术(echo—tracking,ET)检测盆腔肿瘤患者放疗后水肿下肢股动脉弹性功能的改变,探讨电离辐射对下肢水肿产生的意义。方法选择放射治疗6个月后妇科恶性肿瘤患者43例,按下肢有无水肿分为两组:水肿组,共有水肿下肢29条;无水肿组,无水肿下肢57条。选取未接受放射治疗,无下肢水肿患者30例,共60条下肢作为对照组。应用ET技术检测患者股动脉的弹性指标:血管僵硬度参数(B)、压力应变弹性系数(E0)、动脉顺应性(AC)、脉搏波膨大指数(AI)及脉搏波传递速度(PwVB),并进行比较。结果水肿组与非水肿组股动脉弹性指数各指标比较β、Ep、PwV8各值升高,二者差别显著(P〈O.05),AC值降低,二者差别显著(P〈O.05);非水肿组与对照组比较股动脉弹性各指标对比均无统计学意义(P〉0.05),三者AI值对比均无统计学意义(P〉O.05)。结论放射治疗后,水肿患肢股动脉弹性较非水肿患肢减低,显示放疗损伤动脉弹性水肿患肢动脉损伤明显。

Abstract:Objective To explore the value of Etracking (ET) technology in evaluating the elasticity of the femoral ar tery in patients with edema of lower extremity treated with radiotherapy, to explore the bigger vascular injury after radio therapy so as to benefit early diagnosis, early therapy and prophylaxis. Methods 43 patients who received radiotherapy in the fourth Hospital of Hebei Medical University between Dec. 2008 and Dec. 2009 after 6 months to 6 years were slected and divided into edema of lower extremties (29 cases) and nonedema of lower extremties (57 cases) to calculate. The 30 cstiffness parameter (13), pressure strain elasticity modulus(Ep), arterial compliance(AC), argumentation index(AI), and onepoint pulse wave velocity (PWV13) of femoral arteries were calculated automatically by ET technique. Results Com pared with the femoral artery elasticity index of the normal control group, there were no different in , Ep, PWV13, AC, AI ( P 〉0.05) in the nonedema group, but there were significant different in 13, Ep, PWV13, AC ( P d0.05) except AI ( P 〉0.05) in the edema group. Compared with the nonedema group, the data were increased with 13, Ep, PWVI3, while decreased with AC in edema group. Conclusion Compared with the nonedema group, the femoral elasticity index of the edema group was significantly decreased.
