国产厄贝沙坦治疗轻中度原发性高血压的疗效和安全性评价

目的　评价国产厄贝沙坦治疗轻中度原发性高血压的疗效和安全性。方法　采用随机双盲法将 46例轻中度原发性高血压患者分为厄贝沙坦组和氯沙坦组 ,分别接受厄贝沙坦 15 0mg/d或氯沙坦 5 0mg/d治疗 ,4周后若舒张压≥ 90mmHg ,剂量加倍 ,共 8周。治疗前后测量坐位血压 ,记录所有副作用。另有 2 0例病人行开放 2 4小时动态血压监测 ,观察厄贝沙坦对血压波动的影响。结果　厄贝沙坦与氯沙坦均能有效降低血压 ,治疗总有效率分别为 85 .6 %和 82 .6 % (P >0 .0 5 ) ,两组降压程度无统计学意义 (P >0 .0 5 )。 2 4小时动态血压监测显示舒张压和收缩压的降压T/P比值分别为 5 6 %和 6 2 %。厄贝沙坦组具有良好的耐受性。结论　厄贝沙坦 15 0～ 30 0mg ,每日一次是治疗轻中度原发性高血压的安全有效药物。

Efficacy and Safety Evaluation of Domestic Irbesartan in Treatment of Essential Hypertension

Objective To evaluate the efficacy and tolerability of domestic irbesartan used in treatment of mild to moderate essential hypertension. Methods Fourty-six patients with mild to moderate essential hypertension were randomly allocated to receive either irbesartan 150 mg/d or losartan 50 mg/d, which could be increased to 300 mg/d and 100 mg/d if blood pressure control was not satisfactory. Sitting blood pressure was taken and all the side effects were recorded throughout therapy. 24-ambulatory blood pressure was monitored in other hypertensive patients (n=20) . Results Both irbesartan and losartan were effective for lowering blood pressure. 80.6% and 82.6% of the patients in each group were considered to improve in 8 weeks with no significant difference statistically between the two groups. Ambulatory blood pressure monitoring of irbesartan showed that T/P of diastolic blood pressure and systolic blood pressure was 56% and 62% respectively. Both drugs were well tolerated. Only two patients in irbesartan group had mild dry cough. Conclusion Domestic irberartan is very effective in treatment of mild to moderate essential hypertension in daily dose of 150～300mg with good tolerability.
