孕妇脐血中T淋巴细胞亚群的变化与HCMV先天性感染的相关性研究

摘    要:目的探讨人巨细胞病毒(HCMV)先天性感染与孕妇脐血中T淋巴细胞亚群变化的相关性,为HCMV先天性感染的有效预防和免疫治疗提供科学依据。方法随机收集正常产妇36例脐血标本,分离白细胞,然后从分离的白细胞中提取DNA进行HCMV荧光定量PCR检测,同时从36例脐血标本中分别取0.5 ml脐血,采用流式细胞术检测脐血中T淋巴细胞亚群CD3+♂T、CD4+♂T、CD8+♂T的百分比,并计算CD4+♂∕CD8+♂的比值。结果 HCMV先天性感染组与HCMV感染阴性组CD3+♂T淋巴细胞含量分别为(73.11±8.88)%和(71.66±8.59)%,差异无统计学意义(P﹥0.05);CD4+♂T淋巴细胞含量分别为(47.52±8.53)%和(52.476±7.25)%,差异无统计学意义(P﹥0.05);CD8+♂T淋巴细胞含量分别为(25.84±4.75)%和(18.07±4.06)%,HCMV先天性感染组较HCMV感染阴性组高且差异有统计学意义(P﹤0.01);CD4+♂∕CD8+♂比值分别为(1.89±0.45)和(3.026±0.75),HCMV先天性感染组较HCMV感染阴性组低且差异有统计学意义(P﹤0.01)。结论HCMV先天性感染与孕妇脐血中T淋巴细胞亚群即机体细胞免疫应答状态的改变密切相关。

Abstract:Objective To study the relativity between change in T lymphocyte subpopulation and congenital human cytomegalovirus infection in cord blood from pregnant women, so as to provide effective means of prevention and immunotherapy for congenital human cytomegalovirus infection. Methods Thirty-six blood specimens from cord blood of normal puerperants were randomly collected. The peripheral blood mononuclear cells (PBMCs) were separated from plasma, then total DNA was extracted from PBMCs of 36 cord blood specimens. HCMV-DNA was detected by fluorescent quantitative PCR. At the same time, 0.5 ml cord blood was detected by flow cytometry in T lymphocyte subsets including CD3+ ♂, CD4+ ♂ and CD8+♂ ♂ percentage, and the CD4+ ♂ / CDs+ ♂ ratio calculated. Results The CD3+ lymphocyte percentages in the congenital HCMV infection group and the negative group were (73.11 + 8.88)% and (71.66 ± 8.59) %, and there was no significant difference between the two groups (P 〉 0.05). The CD4+ ♂ lymphocyte percentage in the HCMV DNA positive group and the negative group were (47.52 ± 8.53 )% and (52.476 ± 7.25)%, and there was no significant difference between two groups (P 〉 0.05). The CD8+ ♂ lymphocyte percentage in the congenital HCMV infection group and the negative group were (25.84 ±4.75)% and (18.07 ±4.06)%, and the difference between two groups was significant (P 〈 0.01 ). The CD4+ ♂ / CD8+♂ ratios of congenital HCMV infection group and the negative group respectively were ( 1.89 ±0.45) and (3. 025 ±0.75), and the difference between two grouPs was significant (P 〈 0.01 ). Conclusion Congenital HCMV infection is closely related to cells immune response change in pregnant women.
