盆腔手术后腹腔镜下行输卵管结扎术的临床研究

目的探讨盆腔手术后腹腔镜下行输卵管结扎术的安全性、可行性。方法选取有盆腔手术史且自愿要求输卵管结扎的妇女135例,腰硬联合麻醉腹腔镜下行输卵管结扎术：腔镜成功置入腹腔后首先探查盆腔,如无粘连、子宫及附件无异常,采用双点穿刺法行输卵管结扎;如探查盆腔有粘连、子宫及附件有肿瘤或囊肿则采用三点穿刺法,在进行输卵管结扎的同时治疗伴随疾病。结果 135例有盆腔手术史行腹腔镜下输卵管结扎术,手术全部成功,无其他脏器损伤,同时对伴随的疾病进行手术治疗：盆腔粘连松解术,卵巢囊肿、肿瘤剥除术,子宫肌瘤挖除术,卵管系膜囊肿剥除术等。结论盆腔手术后腹腔镜直视下输卵管结扎,创伤小,出血少,手术快,成功率高且能避免其他脏器损失,并能对伴随疾病进行手术治疗,患者易于接受。

Objective Explore pelvic surgery after laparoscopic tubal ligation,the safety,feasibility.Methods Select the voluntary requirements of 135 cases of tubal ligation of women in the history of pelvic surgery,laparoscopic tubal ligation combined spinal-epidural anesthesia: first to examine pelvic endoscopic successfully implanted in the abdominal cavity,such as adhesions,uterine,and accessories without exception,with double pointspuncture tubal ligation;examine pelvic adhesions,uterine,and attachment of tumor or cyst is a three-point puncture during tubal ligation at the same time the treatment of concomitant diseases.Results 135 cases have a history of pelvic surgery laparoscopic tubal ligation surgery was successful in all other organ injury,and surgical treatment of accompanying diseases: lysis of pelvic adhesions,ovarian cysts,cystectomy,uterine fibroidsexcavation technique,tubal mesangial cyst cystectomy.Conclusion The peritoneoscopy pelvic surgery after tubal ligation,trauma,less bleeding,faster surgery,a high success rate and avoid the loss of other organs,and surgical treatment of accompanying diseases,the patient is easy to accept.
