慢性充血性心力衰竭患者血清生长分化因子-15测定的临床意义

目的探讨慢性充血性心力衰竭(CHF)患者血清生长分化因子-15(GDF-15)水平的变化,并探讨其临床意义。方法选择CHF患者60例,测定血清GDF-15水平变化,并与30名健康对对照组进行比较。结果 CHD患者左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、心胸比值和血清GDF-15明显高于健康对照组,左室射血分数(LVEF)明显低于健康对照组(P<0.01);随着NYHA分级的增高,CHF患者血清GDF-15水平相应提高(P<0.05或0.01);血清GDF-15和LVEF呈负相关(γ=-0.658,P<0.01),和LVEDD、LVESD和心胸比值呈正相关(γ=0.588,0.596,0.621,P<0.01)。结论血清GDF-15可反映心功能的状态,并对CHF的诊断和病情判定有重要意义,有望成为CHF的新生标志物。

Clinical significance of serum levels of growth differentiation factor-15(GDF-15) in patients with chronic congestive heart failure

Objective To investigate the changes and clinical significance of serum levels of growth differentiation factor-15(GDF-15) in patients with chronic congestive heart failure.Methods Total of 60 patients with chronic congestive heart failure were matched with 30 healthy controls and the serum levels changes of GDF-15 were detected.Results The left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),cardiothoracic ratio and serum GDF-15 in researched groups was higher than the controls and the left ventricular ejection fraction(LVEF) was lower than the controls(P<0.01);The serum levels of GDF-15 in patients with CHF rose as the NYHA classification went up and it had statistically significance(P<0.05 or 0.01);the serum levels of GDF-15 were significant negative correlation with LVEF(γ=-0.658,P<0.01) and was significant positive correlation with LVEDD,LVESD and cardiothoracic ratio(γ=0.588,0.596,0.621,P<0.01).Conclusions Serum levels of GDF-15 may serve as a good biomarker for reflecting cardiac status and as an important biomarker to diagnose and determine the state of CHF.It may be a new marker for CHF.
