摘 要: | 目的观察预先经不同给药途径给予不同剂量地佐辛用于全麻甲状腺手术患者术后镇痛效果。方法全麻下行甲状腺手术患者80例,随机分为4组(n=20):Ⅰ组生理盐水2ml静脉注射(对照组)、Ⅱ组地佐辛0.1mg·kg^-1静脉注射、Ⅲ组地佐辛0.2mg·kg。静脉注射、Ⅳ组地佐辛0.2mg·kg。肌内注射,均于切皮前10min给药。采用视觉模拟评分法(VAS)及舒适评分法(BCS)评价术后2、6、12及24h时的镇痛效果,并记录不良反应。结果Ⅲ组、Ⅳ组术后2、6、12h时VAS评分低于组及Ⅱ组(P〈0.05);BCS评分4组差异无统计学意义(P〉0.05);Ⅱ组术后不良反应高于其他组(P〈0.05)。结论全麻甲状腺手术患者于切皮前10min肌内注射地佐辛0.2mg·kg^-1可达到满意的术后镇痛效果,且不良反应较少。

Abstract: | Objective To investigate the preemptive analgesia effects of dezocine on postoperative analgesia in patients after thyroid gland surgery with general anesthesia in different methods of administration and different dosage. Methods Eighty patients undergoing thyroid gland surgery with general anesthesia were randomly divided into four groups(n= 20) : group Ⅰ (normal saline 2 ml iv) , groupⅡ (dezocine 0. 1 mg·kg^-1 iv),group Ⅲ (dezocine 0.2 mg·kg^-1iv), group Ⅳ (dezocine 0.2 mg·kg^-1 im). All patients were given the drugs at 10 minutes before skin incision. VAS and BCS scores and side effects were recorded at 2, 6, 12, 24 hours after the surgery. Results Compared with group Ⅰ and Ⅱ , VAS was decreased at 2, 6, 12 hours after surgery in group Ⅲ and Ⅳ (P〈0.05). BCS score were no significant differences in all groups (P〉0.05). The incidence of side effects was significantly higher in Group Ⅲ than that in the other groups. Conclusion The preemptive analgesia with dezocine 0.2 mg-kg-1 injected intramuscularly can provide a satisfactory postoperative analgesia with less side effects. |
