21～40岁人群正常肾小球基底膜厚度的定量研究

目的了解21～40岁人群正常肾小球基底膜(glomerular basement membrane,GBM)的厚度。方法选择年龄为21～40岁,经临床筛选无影响GBM厚度的疾病,且免疫荧光染色、光镜及透射电镜观察均显示为正常肾小球的肾穿刺标本,常规固定、包埋及超薄切片,在透射电镜下进行观察,根据等距离曲线移动原则对每个样本的一个肾小球拍照15张,随机设置测试系统,选择测试线与GBM足细胞侧相交的点作GBM的垂直线,与内皮细胞相交处的距离为GBM截面宽度,进而计算出GBM调和平均厚度。结果 29例样本,GBM调和平均厚度为324.26±47.87(236.49～425.03)nm,男性为345.23±39.94(259.96～394.66)nm,女性为307.23±48.07(236.49～425.03)nm,男、女性两组之间有统计学意义(P=0.031);21～30岁和31～40岁的GBM厚度差别无统计学意义(P=0.861)。结论正常人群中不同性别GBM厚度不同,需区别对待。

Morphometric analysis of the thickness of normal glomerular basement membrane in population ranged from 21 to 40 years old

Objective To assess the thickness of normal glomerular basement membrane(GBM) in population ranged from 21 to 40 years old.Methods Samples from patients without glomerulopathy between 21 and 40 years old were selected.Ultrasections were observed by electron microscopy at magnification of 12,000.Fifteen photos were taken from one section of each sample.Test grids were set by Scandium system.According to the mean wideness of cross-section of GBM we calculated the harmonic mean of GBM for each sample which was referred to as mean ± SD(ranged from minimum to maximum) nm.Results The harmonic mean of all 29 samples was 324.26±47.87(236.49~425.03)nm whereas it was 345.23±39.94(259.96~394.66)nm for males and 307.23±48.07(236.49~425.03)nm for females,respectively,with a significant difference between the above two gender groups(P = 0.031).No significant difference was demonstrated between the 21~30 years group and 31~40 years group(P=0.861).Conclusion The thickness of glomerular membrane is different in normal males and females.Therefore,it should be taken as reference value separately.
